| Summary, etc. | La pathogenèse du lymphome folliculaire est un processus à plusieurs étapes, complexe, qui se développe au cours de « déraillements » successifs des mécanismes moléculaires et cellulaires rythmant l’ontogénie des lymphocytes B. Cette perturbation progressive des mécanismes de diversification des anticorps, de l’homéostasie des cellules B et de leur dialogue bidirectionnel avec les cellules du microenvironnement, se produit au cours d’une longue phase de croissance asymptomatique préclinique, dans laquelle des clones précurseurs tumoraux se disséminent et établissent des niches prétumorales dans les organes lymphoïdes secondaires. Chez le patient, cette évolution sous-clonale « darwinienne » indolente continue à se développer, sous la pression thérapeutique, sur des cycles de plusieurs années, rythmés par des rechutes de plus en plus fréquentes et réfractaires, ou aboutissant à une transformation en lymphome agressif de mauvais pronostic. Les développements technologiques actuels, permettant le décryptage fonctionnel des cellules tumorales et du microenvironnement, devraient donner rapidement de nouvelles clés pour répondre aux grands enjeux thérapeutiques du lymphome folliculaire : identifier les patients à haut risque dès le diagnostic, comprendre les mécanismes spécifiques de leur pathogenèse et les bases cellulaires et moléculaires des cellules fondatrices menant à la rechute. |

In the patient, this indolent “darwinian” subclonal evolution continues to develop, under therapeutic pressure, over cycles of several years, punctuated by relapses becoming more frequent and refractory, or resulting in a transformation into aggressive lymphoma poor prognosis Current technological developments, allowing the functional deciphering of tumor cells and the microenvironment, should quickly give new keys to answer the major therapeutic issues of follicular lymphoma: identify high-risk patients from diagnosis, understand the specific mechanisms of their pathogenesis and the cellular and molecular bases of the founder cells leading to relapse. |
