| Summary, etc. | Pour les patients atteints de lymphomes folliculaires, de nombreux index cliniques ( follicular lymphoma international prognostic index [FLIPI], FLIPI-2, PRIMA-PI), biologiques (profils d’expression génique et mutationnels, ADN circulant), radiologiques (volumes métaboliques par tomographie par émission de positrons [TEP]) ou encore composites (m7-FLIPI) permettent une stratification selon le risque évolutif avant tout traitement par immunochimiothérapie. Le statut évolutif post-thérapeutique déterminé à des temps précoces ( progression of disease within 2 years [POD24], survie sans événement à douze/vingt-quatre mois [EFS12/24], réponse complète à trente mois [CR30], critères TEP de Deauville) permet d’identifier des patients à très haut risque. Toutefois, ces facteurs pronostiques ont encore une portée limitée dans la stratégie thérapeutique et la personnalisation du traitement. L’imagerie TEP constitue certainement l’outil prédictif le plus simple à mettre en œuvre pour proposer une adaptation des traitements postinduction. Une meilleure connaissance de la biologie de la tumeur devrait permettre à terme d’identifier les 10 à 20 % de patients progresseurs rapides et constituer la pierre angulaire de thérapies personnalisées. |

| Summary, etc. | For patients with follicular lymphoma, numerous clinical (FLIPI/FLIPI-2/PRIMA-PI), biological (gene expression and mutation profiles, circulating DNA), radiological (PET metabolic volumes) or composite (m7-FLIPI) indices allow stratification according to evolutionary risk before any immunochemotherapy treatment.The post-therapeutic evolutionary status determined at early steps (POD24, EFS12/24, CR30, Deauville PET criteria) allows to identify patients at very high risk. However, these prognostic factors still have a limited impact on the therapeutic strategy and personalization of treatment. In the short term, PET imaging is certainly the easiest predictive tool to implement in order to propose an adaptation of post-induction treatments. A better knowledge of the biology of the tumour should eventually make it possible to identify the 10 to 20% of patients with rapid progression and constitute the cornerstone of personalized therapies. |
