| Summary, etc. | RésuméLes études cliniques contrôlées randomisées sont considérées comme l’étalon-or dans la médecine reposant sur des preuves. La randomisation occupe une place centrale dans cette stratégie d’évaluation des nouveaux traitements. Cette méthode permet de se prémunir du biais de sélection en assignant au hasard les patients inclus entre les bras traitement et standard. Apparue ces dernières années, la randomisation adaptative bayésienne est une nouvelle technique de randomisation qui a été proposée dans la littérature. L’objectif de cet article est d’aider les cliniciens à comprendre les principes sous-jacents à cette technique mais aussi ses limites principales. Que signifient les termes « bayésienne » et « adaptative » ? Malgré son attrait conceptuel, la randomisation adaptative bayésienne comporte des inconvénients et des contraintes non négligeables. Son utilisation doit être soigneusement examinée, ses avantages et inconvénients potentiels évalués, ainsi que sa complexité. |

| Summary, etc. | Randomized controlled clinical trials represent the gold standard for evidence-based medicine and play a key role in evaluation strategies for new treatments. Patients are assigned randomly to the treatment and control arms, thus avoiding selection bias. In recent years, Bayesian adaptive randomization has been suggested as a new randomization technique. The goal of this article is to help clinicians understand the underlying principle of this new randomization technique, but also its limitations. What do the terms “Bayesian” and “adaptive” actually mean? Despite its conceptual appeal, Bayesian adaptive randomization is associated with a number of non-negligible constraints and drawbacks. Therefore, it should be considered carefully and the advantages and possible constraints should be evaluated, together with an understanding of its complexity. |
