| Summary, etc. | Fondé sur une réinterprétation de données existantes, complémentée par des entretiens, cet article cherche à dépasser la conception académique habituelle du positionnement « entre le marteau et l’enclume » des cadres infirmiers dans les hôpitaux. L’analyse comparative de la mise en œuvre de deux expériences de démarche qualité, l’une en Angleterre l’autre en France, dans un contexte de restructuration en Pôles (France) ou plus anciennement en Clinical Directorates (Angleterre), permet de montrer comment les cadres s’emparent des contraintes gestionnaires et des référentiels de performance. La comparaison montre que, inscrites dans des histoires organisationnelles et professionnelles propres à chaque pays, les cadres se saisissent différemment des marges de man œuvre créées par la tension entre « gouvernementalité » et « gouvernance ». Il apparaît ainsi que, en France, l’enjeu se situe au niveau de la reconnaissance professionnelle collective, que symbolise le rôle tenu par l’Institut de Formation des Cadres de Santé dans l’expérience étudiée. En Angleterre c’est le pouvoir des cadres, au niveau de chaque établissement, qui est en cause ; en atteste la transformation de l’organigramme fonctionnel qui accompagne la mise en œuvre du référentiel « Essence of care ». Chemin faisant les normes de qualité sont ainsi amenées à être redéfinies par les acteurs censés les appliquer. |

| Summary, etc. | Nursing Managers between “governmentality” and “governance”; lessons from the English experienceThrough a reinterpretation of existing evidence, supplemented by interview material, this article seeks to move beyond the common academic conceptualisation of front-line hospital nursing managers in both France and England as “caught between a rock and a hard place”. Comparative analysis of the implementation of two care quality improvement initiatives, one in each country, highlights the particular ways in which these front-line managers, working within Pôles in France or the longer-established Clinical Directorates in England, meet the demands of managerial constraints and performance-measurement mechanisms. The room for manœuvre created by the tension between “governmentality” and “governance” is appropriated differently by managers in each country, according to the opportunity structures established by their distinctive historical, organisational and professional contexts. From this analysis, it emerges that, while collective professional recognition constitutes a major challenge in the French case, involving an essential role for the Institut de Formation des Cadres de Santé, the key change-determining factor in the English case is the capacity for influence which managers are able to generate for themselves within each individual hospital, as illustrated by the transformation of formal structures during the implementation of the “Essence of Care” initiative. In the course of this process, the actors called upon to apply pre-established quality-related norms actually redefine them. |
