| Summary, etc. | L’analyse des prescriptions est un acte pharmaceutique complexe. Plusieurs équipes ont mis en évidence une hétérogénéité des pratiques et proposé des solutions pour la limiter. La première partie de notre étude a pour objectif d’évaluer l’homogénéité des pratiques dans notre établissement. La seconde partie est un retour d’expérience concernant un challenge interne et son potentiel impact sur l’homogénéisation et l’évolution des performances. Méthode. Les pratiques ont été évaluées par la mesure du taux d’intervention et d’acceptation. Ces deux indicateurs ont été comparés selon le statut de l’intervenant (pharmaciens, internes, étudiants 5AHU). L’impact du challenge (trophée pour le meilleur taux d’acceptation) a été évalué en comparant les mêmes indicateurs selon l’intervenant, son statut et la période. Résultats. Les performances observées sont variables selon le statut de l’intervenant. Suite au challenge, entre les trimestres 1 et 2, une homogénéisation a été constatée entre internes et pharmaciens concernant le taux d’intervention mais pas concernant le taux d’acceptation. Pour les deux internes de 1er semestre, les taux d’intervention ont diminué (27 % à 10 % et 20 % à 8 %) et les taux d’acceptation ont augmenté (32 % à 57 % et 29 % à 61 %). Conclusion. Notre étude confirme une hétérogénéité des pratiques dans notre établissement selon le statut de l’intervenant. Des actions ont été mises en œuvre pour la limiter : réunion d’équipe analyse pharmaceutique, e-learning, livret d’aide à l’analyse… Le challenge proposé à notre équipe avait pour objectif d’homogénéiser les pratiques et présentait également un aspect managérial et pédagogique. Comme le suggèrent des modèles tels que Process Communication, une adaptation au profil psychologique permettrait de gagner en efficience : malgré notre faible effectif, notre retour d’expérience semble le confirmer avec plusieurs internes. |

| Summary, etc. | Analysis of prescriptions is a complex pharmaceutical act. Several teams have highlighted the heterogeneity of practices and suggested solutions to limit it. The first part of our study aims to assess the homogeneity of practices in our hospital. The second part is a feedback about an internal challenge and its potential impact on the homogenization and the evolution of performance. Methods. The practices were evaluated by the intervention and acceptance rate. These two indicators were compared according to the status of the health professional (pharmacists, residents, fifth year students). The impact of the challenge (trophy for the best acceptance rate) was evaluated by comparing the same indicators according to the health professional, his status and the period. Results. Differences in performance can be observed according to the status of the health professional. Following the challenge, between quarters 1 and 2, homogenization was observed between residents and pharmacists concerning the intervention rate but not concerning the acceptance rate. For the two new residents, the intervention rates decreased (27% to 10% and 20% to 8%) and the acceptance rates increased (32% to 57% and 29% to 61%). Conclusion. Our study confirms the heterogeneity of practices in our hospital according to the status of the health professional. Actions have been implemented to limit it: pharmaceutical analysis meetings, e-learning, analysis help booklet... The challenge proposed to our team aimed to homogenizing practices and also presented an important managerial and pedagogical aspect. As models such as Process Communication suggest, an adaptation to the psychological profile would make it possible to increase efficiency: despite our small staff, our feedback seems to confirm this with several residents. |
