| Summary, etc. | RésuméÀ ce jour, il existe peu d’études cliniques sur les données « en vie réelle » colligeant les taux de saignement annualisés (ABR), la satisfaction des patients, les types de régime, la consommation en UI chez les patients hémophiles A sous facteur VIII recombinant fusionné avec un fragment Fc (rFVIIIFc, efmoroctocog alpha). Trente patients switchés récemment sous rFVIIIFc suivis au centre de ressources et compétences des maladies hémorragiques constitutionnelles des hôpitaux universitaires de Strasbourg, ont été inclus dans une étude monocentrique prospective post-switch. Ils ont bénéficié d’une étude pharmacocinétique simplifiée qui a retrouvé une demi-vie moyenne de 15,8 heures avec des temps moyens passés au-dessus de 1 %, 2 % et 5 % respectivement de 5, 4 et 3 jours. Aucun inhibiteur du FVIII n’a été mis en évidence chez nos patients. La récupération sous rFVIIIFc, corrélée avec le BMI des patients était de 2,8 % par UI.kg-1,alors que leur demi-vie était corrélée avec les taux circulants d’antigène de Willebrand. Les taux résiduels de facteur VIII et le potentiel de thrombine endogène, paramètre du test de génération de thrombine s’apparentent à ceux des hémophiles modérés/mineurs. La majorité des patients ont été satisfaits de leur switch vers rFVIIIFc avec une diminution des ABR, un seul patient étant en échec clinique. Par ailleurs, les auteurs rapportent une baisse de la consommation de facteur VIII. Le changement thérapeutique au sein de notre cohorte peut donc être considéré comme un succès thérapeutique tant clinique que biologique pour la majorité de nos patients. |

| Summary, etc. | Only a few studies on real-world clinical use of recombinant factor VIII –fusionned with Fc (rFVIIIFc, efmoroctocog alpha) have been performed to date, with data on the annual bleeding rate (ABR), the prophylaxis regimen, and FVIII consumption. The aim of our study was to report the real-world clinical application of rFVIIIFc with additional elements, both biological and clinical. A prospective monocentric study has been conducted in the Haemophilia treatment center (HTC) of the Strasbourg university hospital among the severe haemophilia A patients. Thirty male patients were enrolled in the study. After injection of rFVIIIFc, the average time spent above 5%, 2% and 1% of FVIII was respectively almost 3, 4 and 5 days. The average half-life was 15.8 hours. A strong linear correlation between incremental recovery of rFVIIIFc and weight and between rFVIIIFc half-life and basal VWF:Ag level was observed. FVIII activity measurement for rFVIIIFc showed similar results than those previously published. In the follow-up, residual FVIII activity was on average the one of a mild haemophilia patient, corroborated by the results of endogenous thrombin potential of the thrombin generation assay. In clinical practice, rFVIIIFc was well tolerated and patients were mostly satisfied or indifferent of the switch. A single failure was however noticed. No FVIII inhibitor has been detected. Decrease in FVIII consumption was observed, with reduced or unchanged ABR. The switch was an actual success for almost all of the 30 patients, corroborated by satisfactory clinical and biological results. |
