| Summary, etc. | RésuméLe dosage et le suivi des taux plasmatiques de triglycérides (TG) sont essentiels dans les unités de soins intensifs (USI), notamment pour gérer le risque de pancréatite induite par le propofol. Cependant, l’utilisation de certaines thérapeutiques en USI telle que l’administration intraveineuse d’acide ascorbique, notamment utilisée dans le cadre du Covid-19, pourrait conduire à une fausse diminution des triglycérides par perturbation analytique de la réaction de Trinder. Nous avons rapporté ici le cas d’un échantillon avec des taux sanguins de triglycérides non mesurables dus en partie à des taux plasmatiques élevés d’acide ascorbique. Cependant, des mesures répétées sur le même échantillon quatre jours plus tard ont révélé que le mécanisme d’interférence sur les triglycérides était toujours présent alors que le niveau d’acide ascorbique était très réduit par oxydation. Par conséquent, un mécanisme d’interférence supplémentaire a été suspecté. Après investigations, nous avons constaté que le patient avait également reçu de fortes doses de tacrolimus en raison d’une greffe. Comme des études antérieures ont rapporté que le traitement par tacrolimus entraînait une diminution de l’activité plasmatique mesurée de la lipoprotéine lipase (LPL), nous avons émis l’hypothèse que le tacrolimus ou ses métabolites apparentés pourraient également interférer par inhibition directe de la LPL impliquée dans la méthode d’analyse des TG utilisée. |

| Summary, etc. | Management of triglyceride (TG) levels is essential in intensive care units (ICU), especially to manage the risk of pancreatitis induced by propofol. However, some therapeutics in ICU such as intravenous ascorbic acid protocol, especially used in the context of Covid-19 could lead to false decrease of triglycerides by analytical disruption of Trinder reaction. We report here the case of a sample with unmeasurable triglyceride levels partly due to high plasma ascorbic acid levels. However, repeated measure on the same sample four days later revealed that interference mechanism on TG was still present whereas the level of ascorbic acid was very reduced by oxidation degradation. Hence, additional interference mechanism was suspected. After clinical investigation, we found that the patient had also received high doses of tacrolimus due to a transplant. As previous studies reported that tacrolimus treatment lead to a decrease of the measured plasma activity of lipoprotein lipase (LPL), we hypothesized that tacrolimus or related metabolites could also interfere by direct inhibition of LPL involved in TG analytical method used. |
