| Summary, etc. | A threshold of 50 μg fecal calprotectin per g stool sample (μg/g) is commonly used to diagnose chronic inflammatory bowel disease in adult patients and children over 4 years of age. In younger children, fecal calprotectin values are physiologically increased. For the first time, the objective of our study was to establish reference ranges in newborns for the measurement of meconium calprotectin with an automated assay (Liaison® XL, DiaSorin). A prospective study was conducted in 2022 in the Maternity Unit of the Clermont-Ferrand University Hospital, with the inclusion of full-term newborns without intestinal involvement. The quantitative automated Liaison® XL calprotectin assay was assessed on a panel of meconium samples. In our cohort of 132 term neonates, calprotectin values ranged from 21 to 855 μg/g of meconium (2,5th to 97.5th percentile) and the median value was 194 μg/g. In our cohort, only sex-related differences were observed for calprotectin values. No significant differences were found for the other factors studied (maternal or neonatal). Because of inter-individual variability, a sequential measurement of newborn calprotectin from meconium should be considered. |

| Summary, etc. | Dans le cadre du diagnostic de maladies inflammatoires chroniques de l’intestin, chez l’adulte et l’enfant de plus de 4 ans, le seuil de normalité de la calprotectine fécale est fixé à 50 μg/g de selles. Les concentrations de calprotectine sont décrites comme physiologiquement supérieures chez les enfants plus jeunes, particulièrement durant la première année de vie. L’objectif de notre étude était de définir, pour la première fois, des valeurs de référence de la calprotectine méconiale dosée chez le nouveau-né avec une technique automatisée (Liaison® XL, DiaSorin). Une étude prospective a été menée à la Maternité du CHU de Clermont-Ferrand, avec inclusion de nouveau-nés à terme sans atteinte digestive. Les dosages de calprotectine ont été réalisés sur le Liaison® XL utilisé en routine dans le service de Biochimie. Au sein de notre cohorte (n = 132 nouveau-nés), l’intervalle du 2,5e au 97,5e percentile allait de 21 à 855 μg/g de méconium. À l’exception du sexe du nouveau-né, aucune différence significative des concentrations de calprotectine n’a été retrouvée pour les facteurs maternels et néonataux étudiés. Au-delà des valeurs de référence, la forte variabilité inter-individuelle du biomarqueur doit être prise en compte dans l’interprétation d’un dosage de calprotectine méconiale réalisé chez le nouveau-né. |
