| Summary, etc. | RésuméS’il est un sujet complexe en psychiatrie, c’est bien celui de la prescription médicamenteuse et de l’analyse de ses effets objectifs et subjectifs ; chez l’adolescent notamment, pour lequel l’utilisation des psychotropes est relativement récente, même si elle se généralise rapidement. Le modèle de soins institutionnels développé ici repose sur la théorisation proposée par des praticiens expérimentés : l’espace institutionnel thérapeutique y est métaphorisé comme espace transitionnel. Le médicament est alors conçu comme l’un des objets concrets qui vont permettre une médiatisation du lien et l’instauration d’un objet commun et malléable dans la relation transférentielle établie avec le jeune patient. Nous développons les écueils possibles de la prescription et tentons de mettre en relief les aspects subjectifs et intersubjectifs de celle-ci. Si le médicament doit permettre de contenir une pulsionnalité intra-psychique trop débordante chez l’adolescent, il ne doit pas en revanche être utilisé comme outil de maîtrise ou d’emprise sur lui. Un usage « bien tempéré » des psychotropes permettra de travailler utilement avec le patient sur sa conflictualité interne, au sein de l’institution. |

| Summary, etc. | If there one really complex subject in psychiatry, it is the prescription of medication and the analysis of its objective and subjective effects, particularly in the adolescent, for whom the use of psychotropic drugs is relatively recent, even though it has quickly expanded. The model of institutional treatment developed here rests on the theory proposed by experienced practicians : the institutional therapeutic setting is metaphorized as a transitional space. The medication is therefore conceived of as one of the concrete objects which will allow for mediation of the bond and the establishment of a common, malleable object within the transference relation with the young patient. We discuss the possible pitfalls of the prescription and try to highlight its subjective and intersubjective aspects. If the medication must allow for the containment of intra-psychical drive activity that is too overwhelming for the adolescent, it should nevertheless not be used as a tool to obtain mastery or control over him. A « well-tempered » use of psychotropic drugs would enable one to work usefully with the patient on his inner conflicts, within the institution. |

| Summary, etc. | ResumenSi existe un sujeto complejo en psiquiatría es el de la prescripción medicamentosa y el análisis de sus efectos objetivos y subjetivos en el caso de los adolescentes y particularmente para aquellos cuya utilización de los psicotropos es relativamente reciente así mismo si ello se ha generalizado rápidamente. El modelo de cura institucional desarrollado aquí se apoya sobre la teorización propuesta por los practicantes experimentados : El espacio institucional terapéutico es metaforizado como espacio transicional. Los medicamentos son concebidos como uno de los objetos concretos que van a permitir una mediatización del vinculo y la instauración de un objeto común y maleable en la relación transferencial establecida con el joven paciente. Nosotros tratamos las dificultades posibles de la prescripción y tratamos de señalar los aspectos subjetivos e ínter subjetivos de esta situación. Si el medicamento debe permitir de contenir una pulsionalidad intra-psíquica muy desbordante en el caso del adolescente esta no debe ser atizado como útil de dominación sobre el. Un so bien « temperado » de los psicotropos permitirá de trabajar útilmente con el paciente sobre la conflictualidad interna en el seno de la institución. |
