Summary, etc. | RésuméLe paiement direct imposé dans le cadre de l’Initiative de Bamako a réduit l’accès aux soins pour les plus démunis et les plus vulnérables. De nombreux auteurs recommandent la mise en place de politiques de ciblage social pour rendre les soins gratuits pour ces populations. Cependant, au-delà de son efficacité, le ciblage social pose de nombreux problèmes comme son acceptabilité, la difficulté d’identifier correctement les bénéficiaires et la possible stigmatisation de ces dernières. Cette recherche analyse, selon une approche qualitative, la pertinence sociale de la gratuité sélective des soins accordée aux femmes, aux enfants et aux indigents au Burkina Faso. Cette gratuité sélective des soins est socialement acceptée, même si le choix des groupes est parfois questionné. L’implication des communautés dans la sélection des bénéficiaires joue un rôle majeur dans l’acceptabilité et l’efficacité de ces politiques, en limitant notamment la stigmatisation sociale des indigents. |

Citizen involvement in beneficiary selectionThe Bamako Initiative’s direct payment requirement reduced health-care accessibility for the poorest and most vulnerable. Many authors recommend implementing income-targeting policies to provide free health care to the lowest-income citizens. However, despite being effective, low-income targeting poses many problems, such as social acceptability, the difficulty of identifying the right beneficiaries, and the latter’s possible stigmatization. This research uses a qualitative approach to study the suitability of selective, free health care services for women, children and indigent people in Burkina Faso. It finds that selective, free medical care services are socially acceptable even though the choice of beneficiary groups is sometimes questioned. Involving Burkinabe communities in beneficiary selection plays a major role in making such policies acceptable and effective, and notably reduces the stigmatization of the indigent. |
