| Summary, etc. | Dans cet article, à partir de leur pratique ambulatoire, les auteurs envisagent les différentes modalités et déclinaisons du travail en réseau, qui comprend à la fois le travail avec l’entourage des consultants ainsi qu’avec le réseau des institutions psycho-médico-sociales. L’accent est mis sur le tissage singulier des trajectoires de chacun des demandeurs, tant au sein même de notre service que vers les institutions extérieures. Le patient se trouve donc à l’origine de son réseau. Les partenariats inter-institutionnels nécessitent de clarifier les missions, les types d’accompagnements et les perceptions de chacun afin d’éviter les écueils qui peuvent être liés aux enjeux institutionnels et/ou à la dynamique propre de la personne. |
