Summary, etc. Faced with the danger of ever new states of distress, the act is a privileged solution for adolescents and borderline cases. By favoring the exteriorization of an intrapsychic conflict, by fleeing from a painful psychic development, and by outsourcing ill-contained and dysregulated fantasy activity, acting out can be perceived as a primary treatment modality of symbolization: a sort of sign before being language, capable of being understood, by the clinician, as an attempt at registration. When confronted with patients who appear to escape the possibilities of therapeutic work underpinned by associativity, narrativity, and recall, it is important that these various messenger acts be heard and welcomed by the clinician as fantasy equivalents in order to maintain a psychodynamic listening. This article describes various therapeutics that can be used in institutions in order to treat these patients, tangled up in acts and dependencies.
