Summary, etc. Le développement de systèmes de soins intégrés à base communautaire est en cours au Japon. Ces systèmes visent à aider les personnes âgées à continuer à vivre dans des zones où elles ont l’habitude de vivre. Ce document explique l’approche visée en combinant l’intégration de plusieurs parties prenantes, notamment les systèmes de santé et de soins ainsi que les soignants bénévoles et informels, et en mettant l’accent sur la communauté. Étant donné la complexité d’un tel système, il est important d’établir des priorités locales en fonction des besoins et des capacités existantes, ce qui entraîne des variations dans la manière dont ces systèmes sont développés. L’analyse du système permet d’identifier les principaux fondements sociaux et culturels. En effet, si le rôle des aidants familiaux reste central dans le système, il est possible d’envisager d’apporter également un soutien à ces aidants afin de mettre en place un système de soins intégrés à base communautaire plus efficace et plus résistant.

Summary, etc. The development of community-based integrated care systems is underway in Japan. Such systems aim to support older people so that they can continue living in areas where they are used to living. This paper explains the approach aimed for in combining the integration of several stakeholders, including health and care systems as well as voluntary and informal carers, and achieving a community focus. Given the intricacies of such a system, local prioritization based on needs and existing capabilities is important, which leads to variation in how such systems are developed. Through analysis of the system, key enabling social and cultural underpinnings are identified. Indeed, while the role of family carers remains central to the system, there is a scope to consider expanding support to include such carers, so as to make for a more effective and resilient community-based integrated care system.
