Summary, etc. | RésuméL’enquête de Framingham est généralement considérée comme étant la première grande étude de cohorte moderne d’une maladie chronique. Elle a également joué un rôle majeur dans la reconnaissance des divers facteurs de risque des maladies cardio-vasculaires. À partir d’entretiens réalisés auprès des enquêteurs de Framingham et de documents remontant au projet initial et aux premières années de sa réalisation, cet article montre que les aspects novateurs de la méthodologie de cette étude ont émergé sans réelle intention ni conscience de la part des enquêteurs. Alors que cette enquête avait commencé comme un programme de surveillance de la maladie cardiaque comparable à de nombreuses campagnes précédentes de santé publique menées contre la tuberculose, les enquêteurs de Framingham ont innové au fur et à mesure en fonction des changements de rattachement institutionnel, des exigences de leurs soutiens et des attentes des publics concernés, ainsi que des nouveaux outils conceptuels. Si l’enquête de Framingham a été cruciale dans la découverte des facteurs de risque cardio-vasculaire, elle a été façonnée en retour par ces influences sociales qui conduisaient à une approche plus individualiste et quantitative en matière de compréhension et d’intervention dans les maladies cardio-vasculaires et autres maladies chroniques. |

Summary, etc. | The Framingham heart study is generally considered to be the first large modern cohort study of chronic disease and the major impetus for recognizing different coronary heart disease (CHD) risk factors. Using interviews with Framingham investigators and documents from the planning and early years of the study, I demonstrate that the novel methodological aspects of the study emerged without a great deal of individual agency or consciousness. Beginning as a cardiac control program similar to many older tuberculosis public health campaigns, Framingham investigators innovated in small steps in response to its changing institutional base, the needs of its various supporters and audiences, and new concepts and tools. While Framingham was crucial in uncovering CHD risk factors, the study was itself shaped by many of the same social influences which were leading to a more individualistic and quantitative style of understanding and intervening in CHD and other chronic diseases. |
