Summary, etc. The French m?decine lib?rale system, in which doctors work in independent practice, currently fulfills a public health mission by acting as the first level of access to health care, through the general practitioners (attending physicians). It also makes it possible to have a technical opinion without unnecessary recourse to hospital, via the system of 'corresponding doctors'? for the other specializations. Historically, the clash between the need to organize a health system that is accessible to all, and the independent way in which medicine in France is arranged, has led to a longstanding situation of conflict. However, the future could involve finding ways to ally what is best in this independent system with the public health objectives that must guide the way health care functions. To achieve this, the first level in the system must be maintained. But independent general practice is in difficulty, as much because of medical trade unionism, which has not shouldered its role in the past, as because of ignorance about public health on the part of many decision-makers.
