| Summary, etc. | Depuis 2004, les établissements de santé sont rémunérés en France par un mécanisme de tarification à l’activité (T2A). Cependant, au moment où est instauré ce mécanisme, les tarifs diffèrent selon le secteur, ceux du public étant plus élevés que ceux du privé, considérant que l’hôpital public est plus coûteux. Une politique de convergence tarifaire a été initiée à partir de 2007 afin de contraindre les hôpitaux publics à diminuer leurs coûts de production. Cette convergence a fait l’objet d’une controverse vive entre les représentants des hôpitaux publics, ceux des cliniques privées et le ministère de la Santé. L’article analyse cette controverse en étudiant la façon dont des mécanismes comptables et financiers mettent à l’épreuve les catégories et identités publiques et privées dans le domaine hospitalier et en montrant que cette controverse sur la convergence conduit à penser les objets que constituent les « coûts » des séjours et les « écarts » de coûts entre secteur public et secteur privé. Cette étude montre en quoi les coûts et les écarts constituent des instruments d’action publique singuliers en matière de politiques hospitalières. |

| Summary, etc. | The politics of costs, the politics of varianceSince 2004, private and public hospitals are funded by the social security system through a prospective payment system (PPS). When the PPS was implemented in France, public sector pricing wasset higher than in the private sector for many medical acts. The difference was justified by the fact that public hospitals have higher cost structures. Since 2007, theFrench government has sought to rein in costs in public hospitals by promoting a convergence of prices. This lead to an intense debate between the representatives of the public sector, the private sector and the Ministry of Health. The article analyses this controversy by focusing on how accounting and financial tools redefine the meanings and borders between the “public” and the “private” sectors. It also points out the need to consider “cost” and “cost variances” as policy instruments that contribute to orienting hospital policies. |
