| Summary, etc. | Le lymphome B diffus à grandes cellules est l’hémopathie la plus fréquemment représentée. Une immunochimiothérapie à base de rituximab, cyclophosphamide, doxorubicine, vincristine et prednisone (R-CHOP) permet d’obtenir une guérison dans 30 à 40 % des cas. Les patients réfractaires ou en rechute précoce ont un pronostic très sombre. Le score international prognostic index (IPI) permet d’identifier les patients les plus graves ; un autre facteur pronostique défavorable a été récemment identifié : la sarcopénie. Elle est définie par une réduction de la masse musculaire et des performances et est associée à une réduction significative de la survie globale. Un traitement ciblé pourrait modifier le pronostic de ces patients. |

| Summary, etc. | Diffuse large B-cell lymphoma is the most common hematological disorder. Immunochemotherapy based on R-CHOP provides a cure in 30 to 40 % of cases. Refractory or early relapsed patients have a very poor prognosis. The IPI score can help us to select the most serious patients ; another adverse prognostic factor has recently been identified: sarcopenia. It is defined by a reduction in muscle mass and performance and is associated with a significant reduction in overall survival. Targeted treatment could change the prognosis of these patients. |
