Summary, etc. | La maladie alcoolique du foie regroupe plusieurs entités allant de la stéatose hépatique isolée jusqu’au stade terminal de la fibrose, la cirrhose. Il s’agit d’une maladie au mécanisme physiopathologique complexe faisant intervenir les métabolites toxiques de l’éthanol à l’origine d’un stress oxydatif associé à un état pro-inflammatoire en lien avec une translocation bactérienne et l’activation du système immunitaire. Il existe des interactions complexes entre l’environnement, des facteurs de risques génétiques et le système immunitaire. L’alcool reste à ce jour un problème de santé publique important en lien avec une morbidité-mortalité hépatique et extrahépatique, entraînant un coût non négligeable en dépenses de santé. La compréhension du métabolisme de l’éthanol ainsi que de la physiopathologie de cette maladie permet d’aborder les principales cibles thérapeutiques telles que la corticothérapie associée au N-acétylcystéine dans l’hépatite alcoolique. |

Understanding the metabolism of ethanol and the pathophysiology of this disease makes it possible to address the main therapeutic targets such as corticosteroid therapy associated with N-acetylcysteine in alcoholic hepatitis. |
