| Summary, etc. | L’adénocarcinome gastrique métastatique est une maladie au pronostic sombre dont la survie, encore récemment, n’excédait pas douze mois. Longtemps limité à des protocoles classiques de chimiothérapie cytotoxique, l’arsenal thérapeutique s’est vu élargi ces dernières années grâce à l’apparition de nouvelles molécules. En première ligne, une bi- ou trichimiothérapie comportant un sel de platine et une fluoropyrimidine reste le traitement de référence. Récemment devenu un standard en situation péri-opératoire, la combinaison de docétaxel, oxaliplatine et 5-FU (FLOT/TFOX) est en cours d’évaluation dans une étude de phase III qui pourrait faire évoluer les pratiques. En deuxième ligne, le ramucirumab, antiangiogénique ciblant le récepteur 2 du VEGF, a montré son efficacité seule ou en association au paclitaxel, lui permettant d’obtenir une autorisation de mise sur le marché en France dans cette indication (bien qu’il ne soit pas remboursé). Une chimiothérapie à base d’irinotécan ou taxane peut également être utilisée dans cette situation. En troisième ligne ou plus, le traitement oral par trifluridine-tipiracil est devenu un standard chez les patients en bon état général. Pour les patients ayant une surexpression de HER2, le trastuzumab reste la seule thérapie ciblée ayant montré son efficacité. La preuve de son bénéfice n’a été apportée qu’en première ligne, en association à une chimiothérapie à base de sels de platine. De nouvelles molécules ciblant HER2 tels que le trastuzumab-déruxtécan ont montré des résultats prometteurs qui doivent être confirmés par des études de plus grande envergure. L’immunothérapie en comparaison avec la chimiothérapie a montré des résultats mitigés mais des communications récentes ont suggéré que la combinaison de celle-ci à la chimiothérapie de première ligne pourrait apporter un bénéfice chez des patients sélectionnés ayant une surexpression intra-tumorale de PD-L1. Les résultats complets de ces études de phase III sont attendus prochainement et pourraient faire changer les pratiques. |

| Summary, etc. | Metastatic gastric adenocarcinoma is a disease with a poor prognosis, with a survival at diagnosis not exceeding 12 months until recently. Limited to conventional cytotoxic chemotherapy protocols for a long time, the therapeutic arsenal has been expanded in recent years with the emergence of new molecules. In the first line, doublet or triplet chemotherapy with a platinum salt and a fluoropyrimidine remains the standard treatment. The combination of docetaxel, oxaliplatin and 5-FU (FLOT/TFOX) has recently become a new standard in perioperative setting and is currently being evaluated in a phase III study that could lead to changes in the strategy of care. As a second-line treatment, ramucirumab, an anti-angiogenic agent targeting VEGF receptor 2, has demonstrated its efficacy alone or in combination with paclitaxel, leading to an authorization by the French health authorities (but without reimbursement). Chemotherapy based on irinotecan or taxane can also be used in this setting. In third-line or more, oral chemotherapy with trifluridine-tipiracil has become a new standard of care in patients with good performance status. For patients with HER2 overexpression, trastuzumab remains the only targeted therapy that has demonstrated a survival benefit. Its interest has only been proven in first line-setting in combination with platinum-based chemotherapy. New molecules targeting HER2 such as trastuzumab-deruxtecan have shown promising results that need to be confirmed by larger studies. Immunotherapy compared to chemotherapy has shown unconvincing results, but recent communications have suggested that the combination of immunotherapy with first-line chemotherapy could provide a survival benefit in selected patients with an intratumoral over-expression of PD-L1. Full results from these phase III studies are expected shortly and may lead to changes in the strategy of care. |
