| Summary, etc. | Œsogastric cancers can now benefit from a personalized approach targeting the tumor and its microenvironment, mainly relying on tissue biomarkers determination. In the context of immunotherapy, TPS or CPS score assessment and MMR status are mandatory and EBV status might be useful. These parameters can lead to the use of immune check point inhibitors (anti-PD-1 and PD-L1) which recently revolutionized the therapeutic management of oesogastric cancers. Tumor targeting necessarily encompasses HER2 status which impact on trastuzumab use. In addition, other promising targets are available such as claudin 18, FGFR2 and NTRK, which might, for some of them, belong to the future clinical practice. The aim of this article is to describe the validated and promising oesogastric cancer biomarkers, and their assessment. We also highlight the importance of the management of the biopsy material in this era of precision medicine. |

| Summary, etc. | Les cancers œsogastriques peuvent maintenant bénéficier d’une approche thérapeutique à la carte, reposant en grande partie sur la détermination de biomarqueurs tissulaires, dans le cadre du ciblage du microenvironnement ou de la tumeur elle-même. Dans le contexte de l’immunothérapie, la détermination du score TPS ou CPS pour le statut PD-L1 et du statut MMR sont devenus incontournables et la recherche du statut EBV peut s’avérer utile. Ces paramètres peuvent conditionner l’utilisation des inhibiteurs des verrous de l’immunité (anti-PD-1 et PD-L1) qui viennent de révolutionner le traitement des cancers œsogastriques. Le ciblage de la tumeur s’accompagne nécessairement de la détermination du statut HER2, conditionnant l’utilisation du trastuzumab. Mais d’autres cibles prometteuses existent, telles que la claudine 18, FGFR2 et NTRK, qui pourraient, pour certaines, faire leur entrée en pratique clinique. Dans le cadre de cet article nous nous proposons de rappeler et décrire de façon synthétique, les biomarqueurs des cancers œsogastriques validés et candidats, ainsi que la détermination de leur statut. Nous soulignons également l’importance de la gestion du matériel biopsique à l’ère de la médecine de précision. |
