Summary, etc. As soon as pancreatic adenocarcinoma (PA) is diagnosed, whether at the localised, locally advanced or metastatic stage, it is important to provide supportive care combined with the best possible antitumoral treatment. The psychological impact, in the form of anxiety-depressive disorders (ADD), affects more than one the third patients. It also affects carers, who are directly affected by their loved one’s illness. Psychological symptoms often appear at an early stage in the history of the disease, and in some cases may even be the first symptom or even reveal it. TADs arise from the natural history of PA and its complications, such as pain, jaundice, anorexia and cachexia. However, biological factors may also be involved in triggering anxiety-depressive symptoms, with inflammation playing a major role. Finally, psychosocial as well as medical factors may have an influence on ADD, such as the empathy of the doctor, as perceived by the patient at the time of diagnosis, and of the healthcare team as a whole during follow-up. In this context, early diagnosis and appropriate management of the psychological impact on patients and their carers is a major challenge for their quality of life, as well as optimising adherence to anti-tumour treatments. Increased collaboration between clinicians, clinical psychologists and researchers, with the development of dedicated studies, seems an essential condition for achieving this.

Summary, etc. Dès le diagnostic d’adénocarcinome du pancréas (AP), qu’il soit au stade localisé, localement avancé ou métastatique, la prise en charge par des soins de support associés au meilleur traitement antitumoral est importante. L’impact psychologique, sous la forme de troubles anxio-dépressifs (TAD), toucherait plus d’un patient sur trois. En outre, il touche également les aidants, concernés directement par la maladie de leur proche. Les symptômes psychologiques apparaissent souvent précocement dans l’histoire de la maladie, et dans certains cas même, pourraient en être le premier symptôme voire le révéler. Les TAD découlent de l’histoire naturelle de l’AP avec ses complications, telles que la douleur, l’ictère, l’anorexie et la cachexie. Cependant, des facteurs biologiques pourraient également participer à déclencher des symptômes anxio-dépressifs, avec un rôle majeur de l’inflammation. Enfin, des éléments psychosociaux mais aussi médicaux peuvent avoir une influence sur les TAD, tels que l’empathie du médecin, perçue par le patient lors de l’annonce diagnostique et de l’équipe soignante dans sa totalité lors du suivi. Dans ce contexte, un diagnostic précoce et une prise en charge adaptée du retentissement psychologique des patients et de leurs aidants est un enjeu majeur pour leur qualité de vie, mais aussi l’optimisation de l’adhésion aux traitements antitumoraux. Une collaboration accrue entre cliniciens et psychologues cliniciens et chercheurs, avec le développement d’études dédiées, paraît être une condition essentielle pour y parvenir.
