| Summary, etc. | RésuméComment assurer une prise en charge globale de la fin de vie désirée à domicile par le patient, avec compétence médicale, soutien à la famille et collaboration des professionnels médico-sociaux ? En concevant un réseau comme une unité de soins regroupant différents professionnels et disposant de moyens de coordination afin d’assurer la continuité des soins. Dans cette expérience menée en Haute-Savoie depuis 3 ans, le service infirmier travaille directement en contact avec le malade, le médecin traitant, l’infirmière libérale et la famille. Le service médical intervient sur demande du médecin traitant et de l’infirmière coordinatrice du secteur. Cet article présente une évaluation, à la fois quantitative et qualitative, complétée par des entretiens avec des familles. Globalement, le réseau fonctionne bien. Restent à améliorer l’accès à la formation et la collaboration avec les services d’urgences, notamment pour faciliter l’accès aux dossiers des patients. |

By thinking of a network as a health care team, which includes different professionals, and has the means to coordinate the continuity of care. In this 3-year experience from Haute-Savoie, nursing services work directly in contact with the patient, physicians, the private practice nurse, and the patient’s family. Medical services act on demand from the primary care physician or from the coordinating nurse. This article presents a quantitative and qualitative evaluation, complemented with family interviews. Globally, the network functions well. Points to strengthen include access to training, collaboration with emergency services, and ease of access to patient files. |
