| Summary, etc. | RésuméLes progrès de la médecine et l’allongement de l’espérance de vie sont à l’origine de situations nouvelles. Faut-il traiter un patient âgé dément atteint de cancer ? Le seul fait de poser la question bouscule bien des idées reçues et une attitude très répandue dans la société comme dans le monde médical, l’âgisme. Les mécanismes qui concourent à l’âgisme sont multiples : regard négatif sur la vieillesse, peur de la mort... En termes de valeurs, la priorité donnée par notre société aux valeurs de liberté, d’autonomie et d’efficacité stigmatise particulièrement les patients âgés déments atteints de cancer. Des questions de justice se posent dans la répartition des ressources et les critères d’attribution. En pratique, la prise en charge de tels patients atteints de troubles cognitifs impose de réfléchir aux notions d’information, de qualité de vie et de consentement, éventuellement à l’aide d’un tiers désigné par le patient et de ses directives anticipées. Entre acharnement thérapeutique et négligence, la recherche d’un juste milieu s’impose. L’importance de l’intention, l’influence du contexte, la nécessité de faire un choix et de s’y tenir, le recours à une méthodologie garantissant une certaine valeur éthique à la délibération, la nécessité de réajuster à tout moment et le rappel de la responsabilité individuelle sont des éléments de réponse. Offrir à un patient dément la possibilité d’un traitement, ne serait-ce que l’envisager pour le récuser, c’est faire honneur à sa dignité, c’est le reconstituer dans sa dimension humaine mise en doute par les valeurs prédominantes de notre société que sont la liberté, l’efficacité et le bonheur. |

| Summary, etc. | Medical progress and the prolongation of life expectancy give rise to new situations. Should we treat an elderly dementia patient who has been diagnosed with cancer ? Simply asking this question disturbs conventions, and the widespread social and medical attitude of agism. Multiple mechanisms concur to agism : negative views on old age, fear of death... In terms of values, the priority our society gives to freedom, autonomy, and efficacy represents a particular risk of stigmatisation for elderly dementia patients with cancer. Questions of justice arise in resource allocation and distribution criteria. In practice, clinical management of such patients with cognitive deficits forces reflection on notions such as information, quality of life, and consent, which can be assisted by a designated third party or advance directives. Between therapeutic obstination and negligence, the search for a middle ground is necessary. The importance of intention, the influence of context, the requirement to make a justifiable choice and to maintain it, the use of a methodology which recognises that deliberation has ethical value, the need to continuously readjust, and the focus on individual responsibility, are helpful elements. Offering a dementia patient a chance for treatment, be it only to consider and then recognise reasons to withhold it, is to honour this patient’s dignity, and to reconstitute his human dimension when it is questioned by dominant values such as freedom, efficacy, and happiness. |
