| Summary, etc. | Le service public était depuis le milieu du XXe siècle au cœur de notre choix de société basée sur la solidarité et l’interdépendance sociale. Il a été, au même titre que les assurances sociales, promoteur d’une citoyenneté nouvelle fondée sur la notion de propriété sociale. L’importance grandissante prise, depuis une vingtaine d’années, par les modes de régulation fondés sur le marché est source de conflit avec cette idée originelle du service public. L’hôpital n’échappe pas à cette contradiction, particulièrement depuis la mise en œuvre de la réforme de son financement. La tarification à l’activité et la révolution culturelle qu’elle implique, notamment en termes de logique gestionnaire et de découverte de la notion de rentabilité, risquent de profondément bouleverser la définition de cette notion aidée en cela par une Europe à la pointe du combat libéral. Les conséquences sont à chercher du côté de la sauvegarde de notre contrat social. |

| Summary, etc. | Are public hospital services compatible with activity-based tariff scales... and vice versa ?Since the mid 20th century, public services have been at the heart of a vision for society based on solidarity and social interdependence. Together with social insurances, they have created a new type of citizenship based on social propriety. The growing importance in the past twenty years of market-based systems of regulation is at odds with this original idea of public service. Hospitals are not exempt from this contradiction, particularly since the setting up of the funding reform. The new activity-based tariffs and ensuing revolution, notably in terms of management logic and the emergence of the idea of cost-effectiveness, are in danger of overturning the definition of this notion of public service, all the more so since Europe is at the forefront of the liberal revolution. The safeguard of our social contract is thus at risk. |

| Summary, etc. | ¿ El servicio público hospitalario es soluble en la tarificación por actividad... y viceversa?Desde mediados del siglo veinte el servicio público ocupa un lugar central en nuestro modelo social basado en la solidaridad y la interdependencia social. Al igual que lo fuera la seguridad social el servicio público ha sido un promotor de una nueva ciudadanía basada en la noción de propiedad social. Desde hace unos veinte años, la importancia cada vez mayor de los modos de regulación fundados sobre el mercado origina un conflicto con la idea original de servicio público. El hospital no escapa a esta contradicción sobretodo desde que se ha puesto en marcha la reforma sobre su financiamiento. La tarificación por actividad y la revolución cultural que ello implica, sobretodo en términos de lógica gestionaria y del descubrimiento de la noción de rentabilidad, podrían modificar profundamente la definición de esta noción, precisamente en un momento en que Europa está en pleno combate liberal. Lo que está en juego es la salvaguarda de nuestro contrato social. |
