| Summary, etc. | RésuméL’implantation progressive de la psychiatrie à l’hôpital général, où elle devient une spécialité médicale parmi d’autres, ainsi que la réforme nommée « nouvelle gouvernance » (T2A, quantification de l’activité et budget de pôle en rapport, etc.) modifie le cadre d’un travail orienté traditionnellement en France par le courant de la « psychothérapie institutionnelle ». Je vais dans ce texte rappeler les principes essentiels de la psychothérapie institutionnelle et réfléchir à ses articulations possibles avec la logique managériale et comptable de l’hôpital général. J’insisterai finalement sur l’idée de « négativité », consubstantielle de l’humanité même, touchant à l’essence de l’homme, pour affirmer que cette « négativité » doit faire partie intégrante du soin psychiatrique, bien qu’elle ne puisse pas se quantifier. Je partirai pour ce faire de l’idée selon laquelle l’homme malade est touché dans son essence et que, par conséquent, le soin devra tenir compte de cette dimension non évaluable. |

| Summary, etc. | Institutional psychotherapy in general hospitals: negativity and continuityInstitutional psychotherapy in general hospitals: negativity and continuity.The progressive implantation of psychiatry in general hospitals, in which it becomes one medical speciality among others, as well as the “new governance” reform (T2A, quantification of activity and pole budget adjusted accordingly, etc.) modify the framework of a work oriented traditionally in France by the trend of institutional psychotherapy. The author recalls the main principals of institutional psychotherapy and reflects on possible link-ups with the managerial and accountancy logic of the general hospital. He insists on the idea of “negativity”, consubstantial with humanity itself, touching on the essence of man, affirming that this negativity must be an integral part of psychiatric care, even though it cannot be quantified. The mentally ill person is touched in his essence and all care must consequently take account of this unquantifiable dimension. |

| Summary, etc. | ResumenLa ampliación progresiva de la psiquiatría en el hospital general, donde se convierte en una especialidad médica como las demás, y la reforma llamada « nouvelle gouvernance » (T2A, cuantificación de la actividad y presupuesto de polo en consecuencia, etc.) modifican el marco de un trabajo orientado tradicionalmente en Francia por la corriente de la psicoterapia institucional. El autor describe los principios esenciales de la psicoterapia institucional y reflexiona sobre cuales pueden ser las articulaciones posibles con la lógica administrativa y contable del hospital general. Hace hincapié sobre la idea de « negatividad », consubstancial de la vida humana, que concierne la propia esencia del hombre y afirma que esta negatividad debe ser parte integrante del tratamiento psiquiátrico, aunque no pueda ser cuantificada. El hombre enfermo se ve afectado en su propia esencia y por consiguiente el tratamiento debe tomar en cuenta esta dimensión no evaluable. |
