| Summary, etc. | RésuméCette étude passe en revue les références contemporaines autour du diagnostic d’apotemnophilie – soit le désir d’être amputé –, catégorie principale incluse dans le syndrome Tiric (trouble identitaire relatif à l’intégrité corporelle). Ce dernier est sur le point d’entrer dans la prochaine édition du DSM en tant que trouble spécifique. En contrepoint, une clinique structurale attentive aux variétés des positions subjectives permet de concevoir ce syndrome comme possible modalité d’une suppléance élaborée dans une structure psychotique. Une telle conception ne peut s’argumenter qu’à la condition de prendre en compte la dimension sacrificielle prévalente pour le sujet, comme certitude et nécessité pour le sujet afin de continuer à se situer dans l’existence, donnant à entendre qu’une résolution pacifiante des troubles passe foncièrement par la réalisation du fantasme d’amputation. |

| Summary, etc. | Apotemnophilia in questionThis study reviews the contemporary references about apotemnophilia diagnosis – the desire to be amputed –, main label included in BIID syndrome (Body Integrity Identity Disorder). This syndrome is about to enter in the next edition of the DSM, as a specific disorder. On the contrary, a structural approach attentive to the varieties of subjective positions allows to conceive this syndrome as potential suppleance development in psychosis structure. Such conception couldn’t be supported without paying attention to a prevailing sacrificial dimension for the subject, as a certainty and a need to find a place in the existence, allowing us to hear that a pacifying resolution of their disorder must pass through the realization of amputation phantasm. |

| Summary, etc. | ResumenEl artículo pasa en revista las referencias contemporáneas sobre el diagnóstico de apotemnofilia - el deseo de ser amputado, categoría principal incluida en el síndrome TIRIC - « Trastorno de la Identidad Relativo a la Integridad Corporal ». Este último está a punto de entrar en la próxima edición del DSM como trastorno específico. En contrapunto, una clínica estructural atenta a las variedades de posiciones subjetivas permite concebir este síndrome como posible modalidad de compensación elaborada en una estructura psicótica. Dicha concepción solo puede argumentarse tomando en consideración la dimensión sacrificial prevalente para el sujeto, como certeza y necesidad para el sujeto para seguir situándose en la existencia, dando a entender que una solución pacificadora de sus trastornos pasa obligatoriamente por la realización del fantasma de amputación. |
