| Summary, etc. | RésuméL’objectif général de cette étude prospective à visée analytique était de décrire les itinéraires thérapeutiques des schizophrènes consultant au Service d’hygiène mentale (SHM) de l’Institut national de santé publique à Abidjan. Au terme de l’étude, il ressort que les caractéristiques des itinéraires thérapeutiques étaient en rapport avec les éléments suivants :l’origine mystique était la plus associée à la maladie mentale (43,2 %) ;la conception qu’avaient les sujets et leurs familles de la maladie mentale mais aussi leur niveau économique influençaient la première orientation thérapeutique ;la demande de soins au SHM était de 22,7 % en première intention ;la proportion d’évolutions cliniques favorables de l’état de santé mentale des schizophrènes était de 20,5 % au décours de la première orientation thérapeutique ;chez 38,6 % des sujets, le recours aux lieux de cultes (70,6 %) et aux tradithérapeutes (11,8 %) demeurait malgré le suivi médico-psychiatrique du SHM.L’intérêt de l’étude serait donc que toute activité, tendant à rapprocher les systèmes de soins conventionnels et ceux non conventionnels, bénéficie de la volonté politique des autorités de la santé en vue d’une redynamisation des structures psychiatriques pour toute amélioration de la prise en charge de la santé mentale des populations. |

| Summary, etc. | Therapeutic routes for schizophrenics in AbidjanThe general objective of this exploratory study with an analytical perspective was to describe the therapeutic routes of schizophrenics consulting the Service of Mental Hygiene (SHM) of the National Institute of Public Health in Abidjan. At the end of the study, it was demonstrated that the characteristics of the therapeutic routes had a connection to the following elements :the mystical origin was associated with the mental disease (43.2%),the concept that the patients and their families had, not only of mental disease but also their economic level, influenced the first therapeutic orientation,the request for care from the SHM was 22.7% in first instance,the proportion of favourable clinical evolution of the mental health condition of the schizophrenics was 20.5% with the waning of the first therapeutic orientation,38.6% of the patients who resorted to places of worship (70.6%) and to traditional African medicine (11.8%) remained in spite of the medico-psychiatric follow-up of the SHM.The interest of the study could therefore be that any activity which aimed at bringing the conventional and non-conventional health-care systems closer, would only profit from the political will of the health authorities in order to re-launch the psychiatric structures. This could only improve the management of mental health of this population. |

| Summary, etc. | Itinerarios terapeúticos de los esquizofrénicos en AbidyánEl objetivo general de este estudio prospectivo con enfoque analítico era describir los itinerarios terapeúticos de los esquizofrénicos atendidos por le servicio de higiene mental (SHM) del Instituto nacional de salud pública en Abidyán. Del estudio, se desprende claramente que las características de los itinerarios terapeúticos se relacionaban con los siguientes elementos :el origen místico era el más asociado a la enfermedad mental (43,2 %), la concepción que tenían los afectados y sus familias de la enfermedad mental pero también su nivel económico influían en la primera orientación terapeútica ;la demanda de atención en el SHM alcanzaba un 22,7 % como primera intención ;el porcentaje de evoluciones clínicas favorables del estado de salud mental de los esquizofrénicos era del 20,5% en el transcurso de la primera orientación terapeútica ;el 38,6% de los afectados, seguía recurriendo a los lugares de culto (70,6%) y a los tradi-terapeutas (11,8%) a pesar de la atención médico-psiquiátrica del SHM.Lo interesante del estudio sería pues que cualquier actividad, tendente a acercar los sistemas de atención convencionales y los que no lo son, se beneficie de la voluntad política de las autoridades sanitarias dirigida a volver a dinamizar las estructuras psiquiátricas para mejorar la situación de la población en cuanto a salud mental. |
