| Summary, etc. | RésuméLe rapport « Couty » ne peut se prétendre un rapport comme un autre, c’est pourquoi les auteurs le prennent très au sérieux et en font une analyse complète en le resituant dans son contexte. Cette lecture semble dépasser ce qui y est écrit mais ce serait oublier que la lettre de mission note que la restructuration de la psychiatrie est intégrée à la création des agences régionales de santé et surtout à la loi dite « HPST ». Les auteurs procèdent à une analyse des termes utilisés dans le texte puis de l’architecture envisagée pour la psychiatrie et la santé mentale. Ils montrent comment le projet est sous-tendu par une conception politique de la réponse aux besoins en santé mentale partagée par l’OMS. Les préconisations orienteront la psychiatrie française dans un sens bien différent en rompant l’unité, aujourd’hui en place, entre santé mentale et psychiatrie fondée sur l’approche de la personne à travers la psychopathologie. La psychiatrie infanto-juvénile se voit entièrement diluée dans le social et le médico-social perdant son leadership dans l’organisation et dans la continuité du parcours d’un enfant de la prévention au diagnostic jusqu’au soin. La psychiatrie des adultes est clivée entre ce pôle médico-social et un pôle hospitalier réduit au traitement et à la sécurité. Les images de l’oxymore et de la titrisation (voir définitions page suivante) empruntées à l’actualité chercheront à débusquer les grands axes de pensée qui courent dans ce texte. |

| Summary, etc. | The Couty Report, Sector and infant-juvenile psychiatryThe Couty Report cannot pretend to be a report like any other. For this reason the authors take their title very seriously and make a complete assessment, placing it into its correct context. This interpretation may seem to overlook what in fact has been written in the report but this would be to forget the original aim of the mission which was the restructuring of psychiatry which is linked to the creation of regional health agencies and above all to the “Hospital-Patients-Health-Territory” Law. The authors continue with an analysis of the terms used in the text as well as the envisaged architecture for psychiatry and mental health. They show that the project is supported by a political concept as a response to the needs of mental health shared by the W.H.O. The recommendations would steer French psychiatry in a very different direction by breaking its unity, currently in place between mental health and psychiatry founded on an approach of the individual through psychopathology. Infant-juvenile psychiatry sees itself as losing its leadership within the organization as well as the continuity of the background of a child from prevention of a diagnosis to final treatment. Adult psychiatry is split between the medical-social axis and the hospital axis which has been reduced to treatment and security. The images of the oxymoron and titles borrowed from the current situation attempt to stir-up the main lines of thought that run throughout this text. |

| Summary, etc. | El informe Couty, Sector y psiquiatría infanto-juvenilEl informe Couty no puede presumir de ser un informe más, por lo cual los autores lo toman muy en serio y hacen de él un análisis completo contextualizándolo. Esta lectura parece exceder lo que en él se escribe pero sería olvidar que la carta de misión señala que la reestructuración de la psiquiatría está integrada en la creación de las agencias regionales de sanidad y sobre todo en la llamada ley « HPST ». Los autores se dedican à un análisis de los términos utilizados en el documento y luego de la organización contemplada para la psiquiatría y la salud mental. Aclaran cómo viene el proyecto adosado a una concepción política de la respuesta a las necesidades en materia de salud mental compartida por la OMS. Las recomendaciones orientarán la psiquiatría francesa en un sentido muy diferente rompiendo la unidad, hoy vigente, entre salud mental y psiquiatría en el enfoque centrado en la persona a través de la psicopatología. La psiquiatría infanto-juvenil se ve totalmente diluida en lo social y lo médico-social y pierde su liderazgo en la organización y continuidad del recorrido de un niño desde la prevención pasando por el diagnóstico hasta el cuidado. La psiquiatría de los adultos está escindida entre este polo médico-social y un polo hospitalario restringido al tratamiento y a la seguridad. Las metáforas del oxímoron y de la titulización se toman prestadas a la actualidad tratando de poner al descubierto las directrices que informan este documento. |
