| Summary, etc. | L’évaluation et la prévention des gestes suicidaires constituent un enjeu majeur de santé publique. De nombreuses démarches cliniques ont été proposées pour identifier une activité suicidaire chez un patient. Pourtant, certains passages à l’acte semblent échapper à toute évaluation clinique en présentant un caractère extrêmement brutal. L’article présente le cas de Mme M, une patiente non psychotique ayant fait une tentative de suicide grave. Elle présente le jour même de son geste une amélioration thymique et une baisse de l’activité suicidaire, rendant indétectable l’activité suicidaire. Le soir, elle avale une importante quantité de médicaments sous l’injonction d’une voix. La propulsion de son bras semble altérée, la patiente semble dissociée, sans pensée, sans émotion et sans qu’un événement déclencheur proximal ne soit réellement identifiable. Ces cas ont été identifiés comme des états psychologiques dits kinesthésiques et constituent un intriguant phénomène clinique. Les limites de l’évaluation du potentiel suicidaire sont discutées à la lumière de cette analyse. |

| Summary, etc. | The phenomenon of kinaesthetic psychological states among suicide attempters and limits of suicide assessment: a clinical caseThe evaluation and prevention of suicidal behaviour is a major public health issue. Many clinical approaches have been proposed to identify suicidal activity in a patient. Nevertheless, some acting out seem to escape any clinical assessment particularly patients with an extremely brutal nature. The article presents the case of Mrs. M., a non-psychotic patient who made a serious suicide attempt. She presented the same day of her suicide attempt with a thymic improvement and a decrease in suicidal activity, making any suicidal activity undetectable. In the evening, she swallowed a large amount of drugs following the command of a voice. Propelling her arm seems to alter her, the patient seems dissociated, without any thought, emotion and a proximal trigger event that is clearly identifiable. These cases were identified as so-called kinaesthetic psychological states and constitute an intriguing clinical phenomenon. The limits of suicide assessment are discussed in the light of this analysis. |

| Summary, etc. | La evaluación y la prevención de los gestos suicidas constituyen un reto mayúsculo de salud pública. Se han propuesto numerosos enfoques clínicos para identificar una actividad suicidaría en un paciente. Sin embargo, algunos pasos al acto parecen escapar a toda evaluación clínica al presentar un carácter extremadamente brutal. El artículo presenta el caso de la señora S, una paciente no psicótica con un intento de suicidio grave. El mismo día de su gesto presenta una mejora tímica y un descenso de la actividad suicidaría, que hacen indetectable la actividad suicidaría. Por la noche engulle una importante cantidad de medicamentos bajo un dictado de una voz. La propulsión de su brazo parece alterada, la paciente parece disociada, sin pensamiento, sin emoción y sin que un acontecimiento desencadenante cercano sea realmente identificable. Estos casos han sido identificados como estados psicológicos denominados quien esté pisco y constituyen un intrigante fenómeno clínico. Los límites de la evaluación del potencial suicidario se discuten a la luz de este análisis. |
