| Summary, etc. | RésuméLes notions de rupture et de continuum entre normal et pathologique évoluent, sous l’influence de la quête normative sociétale. En psychiatrie, l’évaluation diagnostique de la pathologie n’échappe pas à cette influence, et mobilise les diverses corps disciplinaires selon des angles de vues théoriques et pratiques différents. La notion de structure psychique illustre le hiatus actuel entre le corps psychologique et le corps médico-psychiatrique. Car si la tendance est actuellement à la classification à la mode américaine, la réalité du terrain conduit à considérer le sens du symptôme dans son acception psychanalytique. Au-delà des carcans restrictifs théoriques, comment articuler ces deux approches, celle du DSM et l’approche de type structurale, d’essence psychanalytique ? Comment mobiliser l’éthique de chaque professionnel, souvent confiné à un paradigme théorique non pas désuet, mais manquant parfois de souplesse. L’avenir du « fou » de nos sociétés en dépend. Il nous paraît pertinent de remettre en question nos schémas perceptifs de la maladie mentale et de poursuivre le mouvement de réhumanisation de la folie initié dans les années 50. |

| Summary, etc. | The concept of structure in questionThe concepts of rupture and continuum between normal and pathological evolve under the influence of a normative societal search. In psychiatry, the diagnostic assessment of pathology does not escape this influence, and mobilizes the various disciplinary bodies according to different theoretical points of view and practices. The concept of psychic structure illustrates the current hiatus between the psychological body and the medico-psychiatric body. However, if the trend is currently according to the American mode of classification, the practical reality leads to consider the sense of the symptom from its psychoanalytical meaning. Beyond the theoretical restrictive constraints, how can we articulate these two approaches, the DSM approach or in essence the structural psychoanalytical approach? How can we mobilize the ethics of each professional, often confined to a theoretical paradigm not obsolete but which sometimes lacks flexibility. In the future the “insane one” of our societies depends on it. It is our duty to question our perceptive diagrams of the mental illness, continuing the movement of re-humanization of madness initiated in the Fifties. |

| Summary, etc. | Las nociones de ruptura y de continuum entre lo normal y patológico evolucionan, bajo la influencia de la búsqueda normativa societal. En psiquiatría, la evaluación diagnóstica de la patología no se libra de esta influencia, y moviliza los diferentes cuerpos disciplinares según enfoques teóricos y prácticas diferentes. La noción de estructura psíquica ilustra el hiato actual entre el cuerpo psicológico y el cuerpo médico-psiquiátrico. Y es que si se tiende actualmente a clasificar a la americana, la realidad del terreno conduce a considerar el sentido del síntoma en su acepción psicoanalítica. Más allá de las trabas restrictivas teóricas, ¿cómo articular estos dos abordajes como el del D.S.M. y el abordaje de tipo estructural, de esencia psicoanalítica? ¿Cómo movilizar la ética de cada profesional, muchas veces reducido a un paradigma teórico no ya obsoleto sino falto a veces de flexibilidad. El futuro del “loco” de nuestras sociedades depende de ello. Nos parece pertinente cuestionar nuestros esquemas perceptivos de la enfermedad mental y proseguir el movimiento de rehumanización de la locura iniciado en los años 50. |
