| Summary, etc. | Devant le risque accru de développer un événement thromboembolique, le risque d’interactions médicamenteuses et les situations complexes des patients atteints de cancer, l’Institut universitaire du cancer Toulouse (IUCT)-Oncopole et le centre hospitalo-universitaire de Toulouse (CHU-T) ont décidé de s’organiser pour faciliter les échanges et les discussions entre les équipes d’oncologie et de médecine vasculaire en créant en 2021 une réunion de concertation pluridisciplinaire (RCP) inter-établissements « Cancer et thrombose ». Au terme d’un travail préliminaire d’organisation, la RCP a débuté en juillet 2021 et se poursuit depuis de façon mensuelle. Après 24 mois de fonctionnement, nous présentons une synthèse sur les motifs d’adressage en RCP, les discussions menées, les difficultés rencontrées et le suivi des préconisations par les praticiens qui nous adressent les patients. En 24 mois, 75 dossiers de patients ont été discutés lors de 20 RCP, majoritairement des femmes atteintes de cancer du sein. Les principales interrogations à l’heure actuelle portent sur le relais héparines de bas poids moléculaire – anticoagulants oraux directs (HBPM – AOD), et sur les indications d’anticoagulation et d’hormonothérapie. Nous sommes aussi interrogés sur des situations individuelles. Les préconisations sont largement suivies (à 68 %) et le principal motif de non-suivi est inhérent au patient (changement d’état clinique, refus de la proposition). Des points d’amélioration ont aussi émergé avec des actions que nous allons mener. Enfin, et à l’aide du dispositif spécifique régional du cancer d’Occitanie, après cette phase de mise en place, la RCP « Cancer et thrombose » évolue pour s’ouvrir aux autres établissements publics et privés de la région Occitanie Ouest. Cette évolution marque une étape importante vers une collaboration plus large et une amélioration des soins aux patients. |

| Summary, etc. | Faced with an increased risk of the development of thromboembolic events, potential drug interactions, and the complexity of cancer cases, our objective was to establish a dedicated organisation to facilitate communication between oncology and vascular medicine. To achieve this, in 2021, we initiated the formation of an inter-institutional cancer and thrombosis multidisciplinary team between the IUCT-Oncopole and Toulouse University Hospital. Following the initial organisation, the cancer and thrombosis multidisciplinary team was launched in July 2021 and has been meeting monthly since then. After 24 months, we summarise the main reasons for referral to this group, the discussions that took place, the challenges that were encountered, and follow-up of recommendations by referring practitioners. Over a span of 24 months, 75 patient cases were discussed during 20 meetings. Most of these cases were women diagnosed with breast cancer. Current focal points of inquiry included the transition between low molecular weight heparin (LMWH) and direct oral anticoagulants (DOAC), as well as the indications for anticoagulation and hormone therapy. Additionally, inquiries regarding individuals were common. Compliance with recommendations was widespread (68 %), and instances of non-compliance were predominantly attributed to patient-related factors. Suggestions for improvement also emerged, and we plan to implement the corresponding actions in the future. Finally, facilitated by the Dispositif Spécifique Régional du Cancer d’Occitanie, following the implementation phase, the cancer and thrombosis multidisciplinary team is progressing towards extending its scope to include other public and private establishments in Onco-Occitanie Ouest. This evolution marks a significant step towards broader collaboration and enhanced patient care. |
