| Summary, etc. | En France, au tournant des années 1990-2000, le refus de transfusion des Témoins de Jéhovah a suscité de vifs débats concernant la hiérarchie des deux obligations imposées aux médecins : le devoir d’assistance à personne en danger et le respect de la volonté du patient. L’article aborde, depuis une perspective de sociologie qualitative, les différentes manières par lesquelles les médecins envisagent aujourd’hui le refus de transfusion. Il s’appuie sur une enquête réalisée auprès de médecins anesthésistes réanimateurs (N=32). Les médecins interviewés ont, pour la plupart, opposé un mode de résolution « à l’ancienne » des tensions inhérentes à la coexistence des deux obligations, aux difficultés actuelles d’élaboration des nouveaux repères cognitifs et moraux de la décision. L’article analyse cette opposition en l’inscrivant dans le cadre des transformations politiques de la médecine et, plus particulièrement, du passage de la tradition clinique paternaliste à la modernité thérapeutique de type participatif. Il présente les trois grandes postures évoquées par les médecins interviewés, pour envisager la conduite de soins dans ce cas de figure particulier. |

Doctors facing refusals to accept transfusionsIn the years 1990-2000, the refusal by Jehova’s Witnesses to accept blood transfusions led to lively debates on how to prioritise the two obligations binding doctors : the obligation to help a person in danger and the duty to respect a patient’s wishes. The article adopts a qualitative sociology perspective, to examine the different ways in which doctors now react to refusals to accept transfusions. It is based on a study of resuscitating anaesthetists (N=32). Most of the doctors interviewed contrasted an “old fashioned” way of resolving tensions inherent to the coexistence of the two obligations, with the current difficulties of developing new cognitive and moral reference points for decision-making. The article looks at this contrast by placing it within the framework of political changes in the medical world and, more particularly, the move from “paternalistic clinical tradition” to “participative therapeutic modernity”. It sets out the three main posititions evoked by the doctors interviewed, to examine healthcare behaviour within this special context. |
