Summary, etc. Cette recherche-intervention porte sur la gestion du médicament au sein du site d’un CHU (Centre hospitalier universitaire) et l’analyse de ses dysfonctionnements organisationnels, ainsi que la mise en place et l’évaluation de projets de changements co-construits entre pharmacie et services cliniques, pour améliorer la performance du circuit du médicament au bénéfice final de la sécurité des patients. Une phase d’écoute préalable des acteurs, suivie d’un dispositif de co-construction de solutions d’améliorations a permis de mobiliser pharmacie et services cliniques. Les actions mises en œuvre et les indicateurs ont amélioré partiellement la coopération. Au-delà, c’est la place de la pharmacie et du pharmacien qui sont ré-interrogés.

Summary, etc. Cooperation between pharmacies and medical services at the hospital and its impact on therapeutic managementThis intervention-research concerns the management of medicines on the site of a CHU (University Hospital Center) and the analysis of the organizational dysfunctions, as well as the setting up and evaluation of change processes, co-elaborated by its pharmacy and its medical services, in order to reach a better performance in the process dealing with medicines and, finally, a better safety for patients. A preliminary phase for listening to actors, followed by a system aiming at co-elaborating improvement solutions has permitted to mobilize actors in the pharmacy and the medical services. The actions implemented and the indicators, have partially improved cooperation. Beyond that, it’s the place of pharmacies and pharmacists which is challenged.
