| Summary, etc. | Le secteur SSR subit une profonde mutation. Engagée par la réforme des autorisations, elle se poursuit par la refonte du mode de financement. La T2A repose sur un idéal de justice, mais sans pouvoir préciser quelle serait la juste rémunération. Elle véhicule également une représentation du secteur SSR, laquelle n’est pas explicitée. L’auteur montre à partir de la valorisation de l’encadrement médical que le modèle suppose une homogénéité qui favorise la médicalisation des soins de suite au détriment de la reconnaissance de la technicité des CRF. Un autre exemple concerne le plateau technique. Enfin, l’analyse des règles par lesquelles l’ENCC répartit les frais de structures révèle qu’elles défavorisent l’hospitalisation de jour (comparativement à d’autres règles), alors que cette activité devrait être reconnue, voire favorisée. Au total, l’auteur déplore une insuffisance de l’analyse conceptuelle. |

| Summary, etc. | And what about rehabilitation?The physical rehabilitation sector is undergoing profound changes. Following the reform of the authorisations, the method of financing the activity is now being reconsidered. The T2A model (along the line of MS-DRG) is based on an ideal of justice but gives no indication as to what the right pricing would be. It also carries with it a representation of rehabilitation which is never clarified. The author uses the cost of medical supervision to demonstrate that the model is based on the concept of homogeneity, which favours the medical staffing of follow-up care and does not acknowledge the technicality of a comprehensive medical rehabilitation centre. Another example is the technical platform and its equipment. Lastly, the analysis of the rules by which the ENCC (French national cost management study) apportions the overhead costs shows how outpatient care is disadvantaged (when compared to other methods of apportionment), whereas this type of activity should be furthered and even favoured. On the whole the author deplores the insufficiency of conceptual analysis. |
