| Summary, etc. | L’objectif de cet article est de réexaminer les limites des études coût-utilité, et plus particulièrement des QALYs, dans le domaine particulier de la greffe d’organes pratiquée à partir de greffons prélevés sur des personnes vivantes. D’inspiration utilitariste, les QALYs constituent un critère d’efficience communément utilisé dans les évaluations économiques réalisées dans le domaine de la santé. Ce critère semble, de prime abord, pouvoir faciliter la prise de décision dans le cadre des prélèvements d’organes. Néanmoins, plusieurs arguments suggèrent que les limites des QALYs sont exacerbées dans le cadre des transplantations d’organes à partir de donneurs vivants. C’est notamment le cas pour le recueil de l’information nécessaire au calcul du ratio coût-utilité, et pour la faible capacité des QALYs à représenter les préférences sociales en la matière. De ce fait, les décisions en matière de greffes à partir de donneurs vivants ne peuvent pas être prises au regard des seuls résultats des études coût-utilité et doivent s’appuyer en pratique sur un panel de critères. |

| Summary, etc. | Organ transplants and interpersonal comparisonsThis article aims at revising the limits of cost-effectiveness analysis, most precisely of QALYs, in the domain of organs transplants with living donors. The QALYs are fully in the spirit of utilitarianism and is the most popular criterion of efficiency when evaluating health policies. Whereas they make easier the medical decision making, we argue that QALYs do not constitute an appropriated tool when applied to live organs transplants. Specifically, the information needed for calculation of cost-utility ratio is rather important. Moreover, QALYs do not properly represent social preferences. Therefore, the decisions about living donor transplantation may not be uniquely taken by the cost effectiveness analysis results and have to be regarded within a multi-criteria framework. |
