| Summary, etc. | Computerization becomes a privileged way for documentation and data management. Nevertheless, computing system has the disadvantage of being likely to fail. Our work had for objectives to develop a manual system as part of the implementation of an alternative system that guarantees the traceability of the chemotherapy circuit in case of computer malfunction, from prescription to administration, and to submit this system to a risk analysis. The latter was set up based on the guide of good preparation practices and the V2010 certification manual of healthcare institutions, developed by the French National Authority for Health, and was described for each step. We used the failure modes and criticality effect analysis method (FMECA) to evaluate the system. Twenty-four failure modes were identified, of which 11 were deemed unacceptable. The main ones were related to steps of prescription, validation and administration. The sum of the criticality indices was 3,711. Corrective actions have been proposed to respond to consistent and efficient support to achieve viable solutions for all failure situations. |

| Summary, etc. | L’informatisation devient un moyen privilégié pour la documentation et la gestion des données. Néanmoins, le système informatique présente l’inconvénient d’être susceptible de tomber en panne. Notre travail avait pour objectifs de développer un système manuel dans le cadre d’une mise en place d’un système alternatif qui garantit la traçabilité du circuit de la chimiothérapie en cas de dysfonctionnement informatique, de la prescription jusqu’à l’administration, et de soumettre ce système à une analyse de risque. Ce dernier a été mis en place en se basant sur le guide des bonnes pratiques de préparation et le manuel de certification V2010 des établissements de santé, élaboré par la Haute autorité de santé, et il a été décrit pour chacune de ses étapes. Nous avons évalué le système par la méthode d’analyse des modes de défaillances, de leurs effets et de leur criticité (AMDEC). Vingt-quatre modes de défaillance ont été identifiés dont 11 ont été jugés inacceptables. Les principaux étaient en relation avec les étapes de la prescription, la validation et l’administration. La somme des indices de criticité était de 3 711. Des actions correctives ont été proposées afin de répondre à une prise en charge cohérente et efficiente pour aboutir à des solutions viables pour toutes les situations de panne. |
