| Summary, etc. | Résumé L'adolescence est une période de transition, de l'enfance vers l'âge adulte. Pour un adolescent qui a un DT1 s'y ajoute une transition du suivi médical. Cette transition médicale est le passage d'un accompagnement par une équipe pédiatrique avec la participation des parents, vers une équipe qui s'occupe des adultes et se passant progressivement de l'intervention des parents. Elle s'inscrit dans un processus d'émancipation ordinaire propre à tous les adolescents. La perspective de la transition doit être annoncée dès le début de l'adolescence ; elle est organisée avec l'équipe pédiatrique. Elle comprend trois phases : la préparation, le transfert et le suivi. |

| Summary, etc. | Adolescence is a time of transition from childhood to adulthood. For a teenager who has T1D, there is also a transition for medical follow-up. This medical transition is the passage from carring by a pediatric team with the participation of the parents, towards a team which takes care of the adults and gradually dispenses with the intervention of the parents. It is part of an ordinary emancipation process specific to all adolescents. The prospect of the transition must be announced from the beginning of adolescence; it is organized with the pediatric team. It includes three phases: preparation, transfer and follow-up. |
