| Summary, etc. | La tomographie par émission de positons couplée à la tomodensitométrie (TEP/TDM) est une technique d’imagerie nucléaire permettant d’obtenir des informations anatomiques et fonctionnelles. La TEP/TDM est de plus en plus utilisée en néphrologie non oncologique étant donné les limitations de l’imagerie radiologique conventionnelle avec injection de produit de contraste chez le patient en insuffisance rénale chronique (IRC). La TEP/TDM après injection de 18F-fluoro-deoxy-glucose (FDG) n’est pas toxique et se caractérise par une sensibilité haute. L’irradiation (∼5mSv) est acceptable. L’IRC n’impacte pas significativement la captation tissulaire du traceur 18F-FDG. Le but de cette revue est de détailler les indications non cancérologiques de la TEP/TDM au 18F-FDG en néphrologie générale et après transplantation rénale. Plus particulièrement, l’intérêt de la TEP/TDM au 18F-FDG dans le diagnostic de l’infection de kyste chez le patient polykystique et dans la mise au point d’une fibrose rétro-péritonéale est abordé. En transplantation rénale, la TEP/TDM au 18F-FDG pourrait aider à l’évaluation diagnostique d’une suspicion clinicobiologique d’un rejet aigu du greffon rénal, et ainsi éviter une ponction-biopsie vaine. Les perspectives de la TEP/TDM au 18F-FDG sont évoquées, y compris les nouvelles approches d’analyse d’image. |

| Summary, etc. | Positron emission tomography combined with computed tomography (PET/CT) is a nuclear imaging technique which provides anatomical and functional information. PET/CT is increasingly used in non-oncological nephrology since conventional radiological approaches after injection of contrast agents are relatively contra-indicated in patients with chronic kidney disease (CKD). PET/CT after i.v. injection of 18F-fluoro-deoxy-glucose (FDG) is not toxic and is characterized by a high sensitivity. The level of irradiation (∼5mSv) is acceptable. CKD does not significantly influence tissue uptake of 18F-FDG. The purpose of the present review aims at detailing the non-oncological indications of 18F-FDG PET/CT in general nephrology and after kidney transplantation. Particularly, 18F-FDG PET/CT appears useful in the diagnosis of cyst infection in patients with autosomal dominant polycystic kidney disease, as well as in the characterization of retroperitoneal fibrosis. In kidney transplant recipients, 18F-FDG PET/CT may help in the diagnostic work-up of suspected acute rejection, thereby eventually avoiding unnecessary kidney transplant biopsy. Perspectives in 18F-FDG PET/CT imaging are discussed, including innovative approaches of image analysis. |
