Summary, etc. Plus de 50 ans après le succès des deux premières transplantations rénales réalisées à Boston et à l’hôpital Necker à Paris, la transplantation rénale est devenue le traitement de choix de l’insuffisance rénale chronique terminale, car elle améliore non seulement la qualité de vie des patients, mais aussi leur quantité de vie. En France, plus de 3700 transplantations rénales sont réalisées chaque année, et plus de 40 000 patients vivent avec un greffon fonctionnel. Ce traitement de l’insuffisance rénale chronique terminale requiert une évaluation précise du futur receveur puis une prise en charge spécialisée multidisciplinaire capable de prévenir, dépister et traiter l’ensemble des pathologies associées et des complications de l’immunosuppression. L’ambition de cet article n’est pas de décrire de manière exhaustive tous les aspects de la transplantation rénale, mais à partir de l’expérience d’une équipe, des données de la littérature récente et des recommandations nationales et internationales, d’essayer de fournir une vision synthétique et chronologique du suivi précoce après la transplantation.

Summary, etc. More than fifty years after the success of the two first renal transplantations in Boston and in Necker hospital in Paris, renal transplantation became the treatment of choice of end stage renal failure, because it improves not only the quality of life of the patients but also their long-term survival. In France, more than 3,700 kidney transplantations are performed every year and more than 40,000 patients are living with a functioning kidney allograft. This treatment of end stage renal disease requires a fine-tuned pre-transplant evaluation and a multidisciplinary post-transplant care in order to prevent, to detect and to treat comorbidities and complications of immunosuppression. The ambition of this manuscript is not to describe in an exhaustive way all the aspects of renal transplantation but starting from the experience of a team, recently published data, and national and international guidelines, to try to provide a synthetic and chronological view of the early post-transplant monitoring.
