Summary, etc. Le syndrome de Cushing est une endocrinopathie fréquente chez le chien. Il touche principalement les animaux âgés et certaines races sont prédisposées. Les signes cliniques, peu spécifiques, sont une polyuro-polydipsie, une polyphagie, une distension abdominale et des troubles cutanés. Plusieurs dosages biologiques peuvent permettre de renforcer une suspicion clinique comme, entre autres, la modification de l’activité de l’enzymologie hépatique. Les tests fonctionnels utilisés possèdent des qualités intrinsèques imparfaites et ainsi il est nécessaire de sélectionner les animaux au préalable afin d’augmenter la probabilité pré-test de ces derniers. L’interprétation des résultats est à réaliser à la lumière des éléments épidémiologiques, cliniques et biologiques. Après avoir confirmé un hypercorticisme, il est important d’en déterminer l’origine (hypophysaire ou surrénalien) afin de proposer la prise en charge thérapeutique la plus adaptée et de déterminer le pronostic. Il est notamment possible de se baser sur les résultats de tests dynamiques, le dosage de l’ACTH endogène et/ou sur des examens d’imagerie.

Summary, etc. Cushing’s syndrome is one of the most common endocrine diseases in dogs. Most dogs are diagnosed older than 6 years and some breeds are predisposed. A suspicion of Cushing’s syndrome is typically based on history and physical examination findings and can be supported by clinicopathological abnormalities. Adrenal function tests are imperfect. Therefore, it is particularly important to perform these tests when there is a high index of suspicion, in order to increase the pre-test probability. Adrenal function tests results should always be interpreted in light of epidemiological, clinical and clinicopathological findings. Following a diagnosis of naturally-occurring Cushing’s syndrome, differentiation between different forms should be considered in order to optimize management strategies and prognosticate. This can be done using dynamic tests, endogenous ACTH assay and/or imaging studies.
