| Summary, etc. | Introduction : Le diagnostic orthodontique est, de façon générale, essentiellement morphologique et le traitement aussi puisqu’il s’agit le plus souvent de modifier la forme des arcades et leurs rapports pour les ajuster à une norme avec, comme finalité, un beau sourire. Les arcades dentaires servent avant tout à mastiquer : c’est même leur fonction principale. Matériels et méthodes : L’auteur décrit dans cet article ce qu’est l’orthodontie (et donc le diagnostic) systémique, en abordant les domaines de la mastication, de la cinématique mandibulaire, des divers mouvements engendrés, entre autres. Ses propos sont illustrés par l’étude d’un cas clinique. Discussion : Les modifications morphologiques que nos appareils imposent aux arcades doivent être adaptées à une mastication physiologique efficace car il en va de la stabilité des corrections orthognathodontiques, y compris pour les corrections chirurgicales. Conclusion : L’adéquation entre forme et mastication, qui se concrétise par la recherche d’une proprioception optimale, doit être notre objectif premier. Le traitement modifie la forme des arcades et leur articulation pour changer les trajectoires des cycles de mastication, lesquelles devraient permettre une meilleure répartition des contraintes sur l’ensemble des dents et assurer la pérennité des corrections. |

| Summary, etc. | Introduction : Orthodontic diagnosis is, in general, essentially morphological and so is the treatment, since it is most often a matter of modifying the shape of the arches and their relationships to adjust them to a standard with the goal of a beautiful smile. The dental arches are primarily used for chewing: it is even their main function. Materials and Methods : In this article, the author describes what systemic orthodontics (and therefore diagnosis) is, by addressing the fields of mastication, mandibular kinematics, and the various movements generated, among others. His remarks are illustrated by the study of a clinical case. Discussion : The morphological modifications that our appliances impose on the arches must be adapted to an efficient physiological mastication because the stability of orthognathodontic corrections depends on it, including surgical corrections. Conclusion : The adequacy between shape and mastication, which is concretized by the search for an optimal proprioception, must be our first objective. The treatment modifies the shape of the arches and their articulation to change the trajectories of the mastication cycles, which should allow a better distribution of the stresses on all the teeth and ensure the durability of the corrections. |
