Summary, etc. The diagnostic approach to facial asymmetries is classically based, in addition to the clinical examination, on a cephalometric analysis carried out on a frontal radiography. Planning of orthognathic surgery requires a three-dimensional (3D) examination to visualize deformities. Nevertheless, the use of cephalometric analysis on 3D imaging remains clinically modest. The main objective of this study was to compare the cephalometric measures obtained from frontal radiographies and 3D imaging in patients with facial asymmetries. Material and Methods : Frontal radiographies and 3D CT scans used for orthognathic surgery planning of 24 patients with asymmetries were selected. Cephalometric points were manually placed on 2D and 3D images by an operator, twice at a 4-week interval. The asymmetry of the subjects was quantified from 12 measurements, then the data obtained in 2D and 3D was compared. Intra-examination repeatability has been calculated. Results : 2D-3D average differences above 2 mm were found in the quantification of mandibular, jugal and maxillary molar symmetry and in the measurements of mandibular symmetry. The 3D measures showed a better repeatability than the 2D measures. Conclusion : The 2D-3D variations observed could have a clinical impact in the diagnosis and management of patients with facial asymmetries. Other studies remain necessary to provide more clinical evidence of the benefits of 3D cephalometry.

Summary, etc. Introduction : L’approche diagnostique des asymétries faciales repose classiquement, en complément de l’examen clinique, sur une analyse céphalométrique effectuée sur une téléradiographie de face. La planification de la chirurgie orthognathique nécessite un examen tridimensionnel (3D) permettant de visualiser les dysmorphies dans les trois dimensions de l’espace. Néanmoins, l’utilisation de l’analyse céphalométrique sur les images 3D reste modeste et le plus souvent non exploitée. L’objectif principal de cette étude était de comparer les mesures céphalométriques obtenues à partir de téléradiographies de face et d’imageries 3D chez des patients présentant des asymétries faciales. Matériel et méthodes : Les téléradiographies de face et les scanners utilisés pour la planification de la chirurgie orthognathique de 24 patients présentant des asymétries ont été sélectionnés. Des points céphalométriques ont été placés manuellement sur les imageries 2D et 3D par un opérateur, à deux reprises à quatre semaines d’intervalles. L’asymétrie des sujets a été quantifiée à partir de 12 mesures, puis les données obtenues en 2D et en 3D ont été comparées. La répétabilité intra-examinateur a été calculée. Résultats : Des différences moyennes 2D-3D supérieures à 2 mm ont été retrouvées dans la quantification de la symétrie horizontale mandibulaire, jugale et molaire maxillaire et dans les mesures de symétrie mandibulaire. Les mesures 3D présentaient globalement une meilleure répétabilité que les mesures 2D. Conclusion : Les variations 2D-3D observées pourraient avoir un impact clinique dans le diagnostic et la prise en charge des patients présentant des asymétries faciales. D’autres études restent nécessaires pour fournir davantage de preuves cliniques des bénéfices de la céphalométrie 3D.
