| Summary, etc. | L’action en santé dans le domaine des substances psychoactives reste un défi pour les intervenants. Au Brésil, elle se heurte à de graves obstacles, notamment à la résistance des usagers, retranchés derrière les systèmes défensifs pathogènes – ce qui devient une source de souffrance pour ces intervenants. À partir de la méthode clinique en psychodynamique du travail, cet article reprend comme objet de ses analyses le rôle de la consommation d’alcool comme idéologie défensive de métier et cherche à dévoiler la dramaturgie ouvrière brésilienne où l’addiction à l’alcool ne se résume pas à un syndrome ou à une maladie. Elle est aussi une résistance-symptôme et si sa syntaxe n’est pas prise en compte par un diagnostic qui différencie ladite résistance et l’alcoolisme, suivi d’une action qui laisse parler le symptôme, on surmontera difficilement le recul de nos politiques de santé. |

| Summary, etc. | Health actions in the area of psychoactive substances remain a challenge for health workers. In Brazil, they face serious obstacles, particularly resistance from users, who are entrenched behind the pathogenic defensive systems – which becomes a source of suffering for these workers. From the clinical method in psychodynamics of work, this paper analyzes the role of alcohol as a defensive ideology and seeks to reveal the brazilian working dramaturgy, in which addiction to alcohol is not just a syndrome or an illness. It is also a symptom-resistance and if its syntax is not considered for a diagnosis that differentiates this form of resistance and alcoholism, after an action that is able to pronounce the symptom, it becomes very hard to overcome the retreat reached by our health policies. |
