| Summary, etc. | Pour pallier les « trous » dans l’offre d’accueil en établissements socio-sanitaires et la mise en défaut de la prise en charge qui y était proposée, depuis une quinzaine d’années se multiplient de microdispositifs censés apporter des réponses individualisées au plus près des besoins des usagers. Les effets non souhaités de la multiplication des acteurs intervenant dans une situation, comme un déficit de coordination, ont ensuite donné lieu à la mise en place de dispositifs intégrés d’accompagnement à même de garantir une continuité et une cohérence du suivi. Après avoir brièvement rappelé le contexte du glissement qui s’est opéré des institutions de prise en charge vers les dispositifs d’accompagnement, l’auteur décrit l’organisation et la grammaire d’action sur lesquelles reposent les dispositifs intégrés. En prenant appui sur une recherche-action menée au sein d’une unité de coaching et de case management pour des jeunes en difficulté dans la transition école-métier, ainsi que sur une autre recherche conduite au sein de services de probation en Suisse romande, il examine le vécu des usagers au sein de ces dispositifs intégrés d’accompagnement. Finalement, il interroge les enjeux de cette évolution au regard de l’hypothèse d’une expérience totalisante de prise en charge. |

| Summary, etc. | In order to make up for the “gaps” in the supply of care in social and health institutions and the failures of the care offered there, we have witnessed over the past fifteen years the proliferation of micro-systems that are supposed to provide individualized responses to meet users’ needs. The undesired effects of the multiplication of actors involved in a situation (e.g., lack of coordination) have then given rise to the establishment of integrated support systems capable of guaranteeing continuity and coherence of care.After briefly recalling the context of the shift from care institutions to support systems, the author describes the organization and grammar of action on which integrated systems are founded. Based on an action-research project conducted within a coaching and case management unit for young people in difficulty in the transition from school to work, as well as on another research project conducted within probation services in French-speaking Switzerland, he examines the experience of users within these integrated support systems. Finally, he examines the stakes of this evolution with regard to the hypothesis of a totalizing experience of social care. |
