| Summary, etc. | Objectif : La prévention périnatale des vulnérabilités médico-psychosociales se traduit par la mise en œuvre de staffs médico-psychosociaux (MPS) dans les maternités. Les objectifs de cette étude étaient de réaliser un état des lieux des staffs MPS de la région Provence-Alpes-Côte d’Azur (PACA) et d’explorer les enjeux éthiques liés aux pratiques des professionnels impliqués dans ce dispositif. Méthodes : Cette étude descriptive mixte a été réalisée en deux phases : une première phase interrogeant les coordonnateurs des staffs MPS de la région via un autoquestionnaire ; une seconde phase consistant en des entretiens semi-directifs auprès des coordonnateurs des staffs MPS du département des Bouches-du-Rhône. Résultats : Pour la première phase de l’étude, le taux de participation était de 83 %. Pour la seconde phase, dix entretiens semi-dirigés ont été menés auprès des coordonnateurs des staffs du département. L’enquête a permis de révéler un « noyau dur » de professionnels participant régulièrement aux réflexions du staff (équipe PMI, psychologue, sage-femme et assistante sociale hospitaliers), ainsi que le respect du cadre réglementaire concernant le partage d’informations, restant malgré tout confus pour les coordonnateurs. Enfin, les enjeux éthiques soulevés par ces situations de vulnérabilité ont été explorés (partage d’information, modalités décisionnelles, représentations). Conclusion : Cet état des lieux a permis de révéler les points forts (dispositif bien implanté sur le territoire, respect du cadre réglementaire, professionnels impliqués…) et éléments à améliorer (repérage anténatal, lien avec la psychiatrie adulte…) concernant ces staffs, et permettra de guider les politiques de santé régionales en faveur de la périnatalité. |

| Summary, etc. | Objective : The perinatal prevention of medicopsychosocial vulnerabilities is implemented through medicopsycho-social staffs (MPS) in maternity hospitals. The objectives of this study were to assess the current status of MPS staffs in the French region Provence-Alpes-Côte d’Azur (PACA), and to explore the ethical issues related to the practices of the professionals involved in this system. Method : This mixed descriptive study was carried out in two steps : the first step was to interview the MPS staff coordinators of the region using a self-administered questionnaire ; a second step consisted of semi-directive interviews with the MPS staff coordinators of the Bouches-du-Rhône department. Results : For the first part of the study, the participation rate was 83%. For the second part, ten semi-structured interviews were conducted with the department’s staff coordinators. The survey revealed a “hard core” of professionals who regularly participate in the staff’s reflections (Maternal-Child Health Services team, psychologist, midwife, and hospital social worker), as well as an important respect of the regulatory framework concerning the sharing of information, which nevertheless remained unclear for the coordinators. Finally, the ethical issues raised by these situations ofvulnerability were explored (information sharing, decisionmaking methods, representations). Conclusion : This survey revealed the strengths (system well established in the region, compliance with the legal framework, professionals involved, etc.) and points to be improved (antenatal identification, links with adult psychiatry, etc.) concerning these staffs, and will help guide regional health policies in favor of perinatal care. |
