| Summary, etc. | Résumé Objectif : Dans le cadre de sa politique de prévention et avant la prise en charge en prestation légale au 1er juillet 2006, la Caisse nationale militaire de sécurité sociale (CNMSS) a proposé la prise en charge de l’ostéodensitométrie pour la détection de l’ostéoporose pour les affiliées de son régime domiciliées dans les régions Auvergne et Rhône-Alpes. Cette étude décrit la prescription dans le cadre de cette prise en charge. Méthodes : Une invitation à contacter leur médecin a été envoyée à toutes les femmes de 60 à 75 ans afin d’apprécier la nécessité de la réalisation d’une ostéodensitométrie pour le dépistage de l’ostéoporose. La prescription du médecin s’accompagnait de l’attestation du respect des critères retenus par la Haute Autorité de santé. Le remboursement de l’acte intervenait sur la prescription renseignée et sur la facturation de l’acte réalisé entre le 1er janvier et le 1er juillet 2006. Nous avons analysé les prescriptions, les actes réalisés le même jour ainsi que le remboursement des traitements indiqués dans l’ostéoporose. Résultats : Une lettre a été envoyée à 2 489 bénéficiaires et 272 ostéodensitométries ont été réalisées dans 121 centres. Au moins un facteur de risque a été validé chez 85,3 % des patientes ayant réalisé l’acte. Un traitement a été instauré chez 54 femmes. Le coût pour la CNMSS comprenant l’acte, les consultations, les actes complémentaires et le premier mois de traitement s’élevait à 70,3 € par femme. Conclusion : La prise en charge financière de l’ostéodensitométrie a favorisé l’accès au soin, mais son évaluation doit tenir compte des variations de pratique.Prat Organ Soins 2009;40(1): 19-24 |

| Summary, etc. | Assessment of bone mineral density testing screening Aim: On the occasion of its prevention policy and before the agreement to bear medical costs in legal benefits on July, 1st, 2006, the Caisse nationale militaire de sécurité sociale (CNMSS) proposed the payment of bone mineral density (BMD) testing for osteoporosis detection for the affiliates of its scheme who are residents of the regions of Auvergne and Rhône-Alpes. This study describes the assessment of this agreement to bear medical costs. Methods: A letter was sent to every female aged 60 to 75 years inviting them to get in touch with their general practitioner (GP) in order to appreciate the necessity of realising a BMD testing for the osteoporosis screening. The validation of the criteria decided by the Haute Autorité de santé was joined to the GP’s prescription. The refund of the act was done on the prescription filled in and on the invoicing of the act realised between January, 1st, 2006 and July, 1st, 2006. The analysis of the prescriptions, the acts realised the very same day, as well as the prescription of the treatments indicated in osteoporosis was realised. Results: 2489 beneficiaries were sent a letter, 272 BMD testings were realised in 121 centres. At least one risk factor was validated for 85.3% of the patients who have realised the act. A treatment was established for 54 women. The cost for the CNMSS, including the act, the consultations, the complementary acts and the first month of treatment came to 70.3 € per woman. Conclusion: The agreement to bear medical costs of the BMD testing favoured the access to care but its evaluation has to take into account variations of practice.Prat Organ Soins 2009;40(1): 19-24 |
