Summary, etc. | RésuméLes systèmes sanitaires occidentaux sont fréquemment considérés comme étant en crise. Par rapport à cette situation, le premier article cherche à préciser la façon dont le problème se pose et à revenir sur les difficultés d’élaborer des solutions pertinentes. Le second article cherche à définir une stratégie capable de faciliter les changements nécessaires.Le premier article, « Améliorer la prise en charge des maladies chroniques - 1 : Problématique », vise à formuler une problématique liée en particulier, à l’évolution de la morbidité sur les dernières décennies. Pour y parvenir, les points suivants sont successivement abordés :en premier lieu, nous cherchons à expliquer l’écart d’efficacité de la prise en charge, entre les maladies aiguës et les maladies chroniques. Dans cette perspective, nous examinons comment le principe d’autonomie de la décision médicale qui structure le système sanitaire, et le caractère modulaire de ce système, donnent naissance à un paradoxe selon lequel, les progrès de l’efficacité qui bénéficient principalement aux maladies aiguës, finissent par affecter l’efficacité de l’ensemble de système ;en second lieu, nous soulignons les carences de l’organisation du système, en précisant les causes et conséquences de la prédominance de l’hôpital et d’une médecine ambulatoire à la traîne ;en troisième lieu, nous analysons, à partir d’exemples français et suisses, pourquoi les mesures retenues pour améliorer la prise en charge des pathologies chroniques, ont souvent des résultats limités. Summary, etc. | Western health systems are often said to be in a state of crisis. The first article explores how this problem can be defined in detail and reviews the difficulties in finding relevant solutions. In the second article, the authors seek to define a strategy to facilitate the implementation of the necessary changes.In the first article, “Improving chronic disease management - 1. the problem”, the issue is explored as it relates in particular to the evolution of morbidity over long periods of time. The following points are successively discussed:an explanation is first sought for the difference in the efficiencies of managing acute conditions and chronic disease. The authors examine how the principle of autonomy of medical decision-making which is at the basis of western health systems and the modularity of these health systems give rise to a paradox, whereby progress in the efficiency of acute disease management ends up negatively affecting the system as a whole;the authors then address the organizational shortcomings of the system and describe in detail the causes and consequence of the predominance of hospital medicine and the lagging position of ambulatory medicine;in the third part of the article, specific examples in France and Switzerland are analyzed to reveal some of the reasons for the often mediocre results obtained with measures implemented for better management of chronic pathologies.
