| Summary, etc. | RésuméCet article présente une programmation régionale du système de santé comportant une approche d’intégration des soins et une approche populationnelle. De plus en plus, la notion de responsabilité populationnelle est présente dans la plupart des réformes des systèmes de santé. Ce concept suppose la convergence des efforts à l’égard de la santé et du bien-être d’une population donnée et à l’égard de déterminants de la santé incluant l’accès à des services de qualité pour cette population. Il s’établit un consensus sur la nécessité d’une relation directe et durable entre les prestataires et la population du territoire desservi. Ceci permet de tenir compte des contextes personnels et sociaux des patients et de leur famille, ainsi que d’assurer la continuité des soins dans le temps et d’un service à l’autre. C’est dans cette optique que la Programmation régionale de services ambulatoires (PRSA), initiative d’intégration régionale des soins basée sur une approche populationnelle, présente un intérêt dans la planification de l’offre de soins et services. Cet article vise donc à décrire la PRSA, à la modéliser et à analyser son bien-fondé, c’est-à-dire la plausibilité des mécanismes causaux présumés. Prat Organ Soins. 2011;42(4):235-244 |

| Summary, etc. | This article aims to present a regional initiative which combines integration and populational approach in healthcare organisations. The majority of healthcare reforms insist on the populational responsibility to optimize the services impact on population’s health. This concept is based on the conviction that health is collective, therefore necessary to maintain and develop. In addition, there are more and more consensus on the necessity to establish a direct and durable relationship between care providers and communities. Thus, a way to be able to considerate the patient’s personal and social contexts and to ensure continuous care.This article based on the Regional Programmation of ambulatory services (PRSA) of Laval, analyzes this program as an innovative solution concerning the introduction of populational approach in healthcare.
