Summary, etc. | Resumé Les enfants souffrant de pathologie narcissique présentent fréquemment des troubles du comportement et une agressivité qui rend leur insertion sociale et scolaire difficile. À côté de l'approche psychodynamique, la prescription de médicaments psychotropes peut être une aide précieuse. Les auteurs présentent une revue de la bibliographie concernant les principaux psychotropes non neuroleptiques qui peuvent être utilisés dans les troubles psychiatriques où apparaissent souvent des conduites agressives. Des études contrôlées ont montré que l'agressivité explosive répondait assez bien au lithium, que l'agressivité des enfants hyperactifs pouvait être traitée par les psychostimulants et que la clonidine était efficace pour l'agressivité dans l'hyperactivité et le syndrome de Gilles de la Tourette. D'autres médicaments semblent prometteurs dans des études ouvertes mais n'ont pas encore fait la preuve de leur efficacité : le valproate de sodium, les IRSR et les b-bloquants. Une partie de ces produits n'ont pas d'autorisation de mise sur le marché (AMM) en France dans ces indications, ce qui implique de réaliser une démarche clinique rigoureuse afin d'affiner notre connaissance des modes d'action des psychotropes. |

Summary, etc. | Abstract Children suffering from a narcistic pathology often present behavioural disorders and aggressiveness that renders their social and educational insertion difficult. Beside the psychodynamic approach, the prescription of psychotropic drugs can be of precious help. The authors presents an overview of the literature with regard to the principle non-neuroleptic psychotropics that can be used in psychiatric disorders during which aggressive behaviour often occurs. Controlled studies have shown that explosive aggressiveness responds fairly well to lithium, that the aggressiveness of hyperactive children can be treated by psychostimulating agents and that clonidine is effective in treating the aggressiveness in hyperactivity and Gilles de la Tourette's syndrome. Other drugs appear promising in open studies but have not yet demonstrated their efficacy : sodium valproate, IRSR and b-blockers. Some of these products have not obtained marketing authorisation in France in these indications, which implies that rigorous clinical trials are required to perfect our knowledge of the modes of action of psychotropics. |
