Summary, etc. | Resumé Le diagnostic d’hyperactivité selon le DSM-IV doit être différencié des symptômes d’agitation ou d’inattention qui s’intègrent dans le cadre d’un trouble psychiatrique plus global tel que le trouble de la personnalité et doit être différencié d’une agitation secondaire à des conditions environnementales défectueuses. De nombreuses recherches, rarement citées, montrent pourtant que l’hyperactivité, y compris la forme dite essentielle, peut être considérée comme une manifestation symptomatique d’un processus psychopathologique (de type limite ou prépsychotique) silencieux, pauvre en manifestations mentales, ce qu’on peut dénommer par « psychopathologie blanche », convoquant la clinique du négatif et se manifestant par une hyper-expressivité caractérielle, comportementale, agie palliant aux défaillances de la symbolisation. Une illustration clinique est présentée. |

Summary, etc. | Defense for a psychopathological approach of hyperactivityThe diagnosis of hyperactivity according to DSM-IV must be differentiated from the symptoms of agitation or carelessness which are integrated within the framework of a more total psychiatric disorder such as the personality disorder and must be differentiated from a secondary agitation under defective environmental conditions. Many research, seldom quoted, however shows that hyperactivity, including the form known as essential, can be regarded as a symptomatic demonstration of a psychopathological process (of type limiting or prepsychotic) quiet, low in mental demonstrations, which could be named by « white psychopathology », convening the negative clinics and appearing as characterial, behavioral, acted hyper-expressivity, mitigating the failures of symbolization. A clinical illustration is presented. |
